Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order. Violation(s) A permitted rooming house inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 24 Hours .500 Unit 13: There is evidence of water leakage in the apartment's ceiling. Have a licensed plumber find and repair leak. Scrape off and repaint water damaged paint. Owner has 24 HOURS to start making a good-faith effort to correct violation. .351 Unit 13: The burners on the stove do not work. Either repair burners or replace 7 Days stove with one with working burners and oven. Owner has 7 DAYS to start making a good-faith effort to correct violation. .150 Unit 13: Bathtub wall next to faucet has split open and has exposed hard-edged 7 Days sides. Repair bathtub wall so that it is restored to being a smooth, easily cleanable surface with no defects. Owner has 7 DAYS to start making a good- faith effort to correct violation. .351 Unit 13: The toilet's lift chain is broken and cannot be flushed using the toilet 24 Hours handle. Repair toilet so that it can flush using the handle as intended. Owner has 24 HOURS to start makina a aood-faith effort to correct violation. .550 Unit 13: Mice droppings found on floor in kitchen. Owner is to hire a 24 Hours professionally licensed exterminator and have the exterminator exterminate the apartment and all adjacent walls. Any holes leading into the apartment must be plugged to prevent re-entry from mice. Owner must send extermination report to the Salem Board of Health after the extermination has taken place. Exterminator is to return as often as needed to rid the property of mice. Owner has 24 HOURS to start makin ood-faith effort to correct violation. 500 Unit 11: Ceiling is covered with flaking and peeling paint. Scrape off flaking and 7 Days peeling paint and repaint ceiling. Owner has 7 DAYS to start making a good-faith effort to correct violation. i .351 Unit 24: The toilet's lift chain is broken and cannot be flushed using the toilet 24 Hours handle. Repair toilet so that it can flush using the handle as intended. Owner has 1 24 HOURS to start making a good-faith effort to correct violation .500 Unit 41 (Smaller Side): (NOTE: The entire top floor of the property is made up 24 Hours of two apartments with a shared kitchen. Both are named Unit 41.) Bedroom window's top sash cannot stay up on its own. Repair or replace sash so it can open and close with ease and stay in the position it is placed at on its own power. Owner has 24 HOURS to start making a good-faith effort to correct violation. N/A Owner has 30 DAYS to correct all violations listed in this report.
