In accordance with Chapter 111,Sections 127A and 127B of the Massachusetts General Laws, 105 CMR 400.000;
State Sanitary Code, Chapter 1: General Administrative Procedures and 105 CMR 410.000: State Sanitary Code,
Chapter 11: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 14
Nichols Street Unit 2 permitted by occupant Patricia Garcia conducted by Jeffrey Barosy, Sanitarian on
December 21,2020 at 1:30pm.
Notice:if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn. You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports, orders and other
documentary information in the possession of this Board, and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted: _
7 Days .452 1. Stairways at the rear of the property were found covered in snow.
Egresses and walkways leading to and from the property's entryways
are to be cleared of snow. Owner has 7 DAYS to start making a
good-faith effort to correct violation.
.351 2. Bathroom sink basin is loose and not affixed cupboards and wall.
24 Hours Repair bathroom sink basin so it is properly affixed to cupboards and
wall. Owner has 24 HOURS to start making a good-faith effort to
correct violation.
.150 3. Bathtub's basin's lining has chipping and peeling paint. Repair or
7 Days replace bathtub basin so that there is a smooth, easily cleanable
surface without chipping and peeling surfaces. Owner has 7 DAYS to
start making agood-faith effort to correct violation. _
.351 4. Valve handle that turns on bathtub's water is broken and detached
24 Hours from valve. Repair bathtub's valve handle so that water can be turned
on and off with ease. Owner has 24 HOURS to start making a good-
faith effort to correct violation.
551 5. Bathroom window lacks screen. Provide screen for bathroom window.
7 Days Owner has 7 DAYS to start making a good-faith effort to correct

480 6. Kitchen window farthest to the left has a broken lock. Repair or
7 Days replace lock. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.500 7. Kitchen windows farthest to the left and in the middle have screens
24 Hours and gaps covered up with duct tape. Repair windows so gaps no
longer exist, and screens fit in window as intended without any aide.

.351 8. The base of the kitchen sink's faucet is detached from the counter.
24 Hours Repair or replace faucet so it is properly attached to the counter.

24 Hours licensed electrician investigate and fix cause of issue. Owner has 24
HOURS to start making a good-faith effort to correct violation.
.500 11. Front door leading into the apartment has a gap at the bottom, a
7 Days crack in the area next to its doorknob, and a hole near the top. Have
door either repaired or replaced. Owner has 7 DAYS to start making a
_good-faith effort to correct violation.
.551 12. Hallway leading to stairs to upper floor from kitchen has a window
7 Days with a tom screen. Repair or replace window screen. Owner has 7
DAYS to start making a good-faith effort to correct violation. _
.500 13. Living room ceiling has signs of water damage which include bubbling
24 Hours and discoloration. Find source of leak and have it fixed by licensed
contractor. Removed water damaged paint from ceiling and repaint
effected areas. Owner has 24 HOURS to start making a good-faith
_ effort to correct violation..500 14. Back hallway has plexiglass covering_broken window. Repair window
24 Hours so it operates as intended. Owner has 24 HOURS to start making a
_ good-faith effort to correct violation.
.351 15. Both of stove's front burners have only their outer ring portions
24 Hours operating when turned on while the middle portions do not function.
Repair stove's front burners so that all portions operate as intended.
Owner has 24 HOURS to start making a good-faith effort to correct

N/A 17. NOTE: Occupants do not have ready access to their circuit breakers

N/A 18. NOTE: Thermostat for apartment is not located in an—area—that
N/A occupants can readily access. Owner is responsible for maintaining
temperatures between 64*F and 78*F(11:01 pm to 6:59am)and 68*F
and 78*F(7:00am to 11:00pm)within the apartment during the
heating season (September 15'to the following June 15tn) _
