Subject: Halstead Salem Station: 72 Flint Street, 67-71 Mason Street
To Whom It May Concern,
This letter is to inform Authorities Having Jurisdiction in the City of Salem Massachusetts that there has been a change
within the engineering team on the construction project for the subject property. Moving forward, Consulting
Engineering services and most Engineer Of Record(EOR)duties for Halstead Salem Station located at 72 Flint Street and
67-71 Mason Street will be completed by Elevated Design Inc.
The Consulting Engineering Team will consist of
• Mark Schow, Mechanical PE (49223)for all Mechanical, HVAC,and Plumbing issues and Final Affidavits.
• Ervin Kulenica, Electrical PE(49248)for all Electrical and Fire Alarm issues and Final Affidavits.
• Luis Fernandez, Fire Protection PE (39913)for all Fire Protection and Fire Alarm issues. Note that the Engineer
of Record for Fire Protection will be the Sprinkler Contractor's Engineer; however, Luis will be available for any
trouble shooting or other concerns.
Moving forward, in accordance with Code Massachusetts Regulations,the team above,or our designees,will perform
the necessary professional services for the construction administration phase of the project and will be present at the
construction site on a regular and periodic basis.
At the completion of construction, in accordance with the requirements of 780 CMR and the design documents
approved as part of the building permit and that the team above or our designee: 1. Have reviewed,for conformance to
this code and the design concept, shop drawings,samples and other submittals by the contractor in accordance with the
requirements of the construction documents. 2. Have performed the duties for registered design professionals in 780
CMR Chapter 17,as applicable. 3. Have been present at intervals appropriate to the stage of construction to become
generally familiar with the progress and quality of the work and to determine if the work was performed in a manner
consistent with the construction documents and this code.
