OVERVIEW
In accordance with NFPA 241, the document herein describes the site specific plan of action
and safety management procedures that will be in place through the duration of the
construction project at 11 Dodge Street in Salem, Massachusetts. This plan is intended for the
sole use of this particular site and shall be coordinated with the local fire department. The
following sections outline our fire safety prevention and preparedness in case of a fire or
catastrophic event.

EGRESS & EVACUATION PLAN
In the event of a fire or catastrophic event there will be a single long blast (5 seconds) activation
from an air horn from one of the various locations on the site. An air horn will be posted at the
central fire extinguisher location adjacent to the elevator at each floor.
During initial site orientation all workers will be notified of the horn blast protocol, which will
be to leave the site immediately in an orderly fashion via one of the three egress stairs. They
will be directed to the area of refuge that is closest to their location closest to Gate A or Gate D
as depicted on the attached site plan. Each subcontractor on site will be responsible for doing a
roll call for their employees and report the roll call to the Opechee Construction Corporation
Field Project Manager. The evacuation plans will be posted at a central location on each floor
denoting the quickest route out of the building as well as the area of refuge.
The Opechee Construction Corporation and the project subcontractors will maintain al l egress
paths and evacuation routes by removing debris, equipment or other obstructions, including
snow in order for a safe evacuation if required.
Jeff Daigneault, the Field Project Manager, will be the designated person on site to direct
workers to the area of refuge, notify the emergency services and direct incoming emergency
personnel and vehicles.

FIRE EXTINGUISHER, FIRST AID KITS AND AIR HORN LOCATIONS
Fire Extinguishers:
Located at each egress stair on each floor and adjacent to the elevator at each floor. In
addition, per section 906 there will be at least one multipurpose portable fire extinguisher
located at a central location on the roof near the elevator shaft. Fire extinguishers will be wall -
mounted if applicable or will be placed on a stand if required.
First Aid Kits:
An approved first aid kit will be at the Opechee Construction Corporation field trailer.
Air Horns:
One per floor located at the same location of the fire extinguisher adjacent to the elevator as
well as at the Opechee Construction Corporation field trailer.

AUDIBLE NOTIFICATION
- One long blast (approximately 5 seconds) will be the signal for site evacuation.
- 3 short blasts (approximately 2 seconds each) will be the signal for first aid

TEMPORARY HEAT DETECTION
During construction, after combustible materials (wall construction) has begun, heat detectors
can be installed along the corridors of each floor if required. These can remain in place until
finish ceilings are installed. This requirement should be confirmed by the local fire department.
There will be no permanent smoke detection or active sprinkler systems prior to substantial
completion.

TEMPORARY HEATING EQUIPMENT
Temporary Gas fire heaters will be installed approximately November 2019 and removed
around April 2020. Before putting into operation an onsite inspection by city officials will take
place.

WASTE DISPOSAL
A centrally located dumpster will be provided on site by Opechee Construction Corporation for
the use by all the project subcontractors. The dumpster permit has been procured through the
local fire department and will be a standard construction waste dumpster. All hazardous
materials (if any) on site shall be removed by the contractor responsible for their utilization.
Each subcontractor shall maintain the MSDS for all items on site to ensure that no hazardous or
self-igniting items are placed in the dumpster.
At no time shall combustible material be accumulated in the building. Opechee Construction
Corporation is committed to maintaining a clean and clear construction site where all debris
shall be removed from the site each work day. We have reinforced th is plan by providing the
centrally located dumpster and trash chutes to encourage continuous cleanup. There will be no
open burning on site and at no time shall any debris or rubbish be burned on site.

HOT WORK
All hot work will be done per OSHA standards and City of Salem, MA Fire Dept. Requirements.
As most of our piping is done with PVC and CPVC, hot work is minimal or non-existent once the
steel erection is complete. We will work with the local fire department and steel erector for the
hot work permit associated with cutting and welding steel.

FLAMMABLE AND COMBUSTIBLE LIQUIDS
At no time shall flammable or combustible liquids be stored within the footprint of the building.
During steel erection operations, all liquids shall be secured in accordance with section 3404.
The storage shall be lockable with appropriate egress and kept away from combustible
vegetation.

WATER SUPPLY FOR FIRE PROTECTION
Currently there are fire hydrants located in multiple locations around the perimeter of the site
as per below.
The fire protection contractor will install temporary standpipes at the stairwells all the way to
the roof in order to be utilized for firefighting during the construction phase of the building.
These standpipes will need to be manually charged by the fire department upon arrival on site
and will be equipped with fire department connections at all floors.

ACCESS FOR FIRE FIGHTING
Access into the project site all the way to within 25 feet of the building shall be maintained at
all times. Snow removal will be by Schulte Inc throughout the winter and all gates will remain
accessible. The fire hydrants will be marked and shoveled clear for access.

CONCLUSION
We are committed to having and maintaining a clean and safe site for our workers, our
subcontractors, municipal personnel, designers, third party agents and emergency personnel. It
is our intent to review this program, as well as any potential changes or modifications, with the
local jurisdiction and to be in constant communication throughout the duration of this
construction project.
