In November the Salem Board of Health sent you an application for your 2020 Food Permit via email.
Included in that email was a cover letter stating that you were required to return the application by
December 13, 2019.
To date we have not received your application or application fee.
You are operating your establishment without a permit in violation of the State
Sanitary Code Chapter X: Minimum Sanitation Standards for Food Establishments,
105 CMR 590.000, Section 8-301.11.
Therefore ou are ordered to close your establishment on Monday. Februal-N, 17, 2020 if the
application, required permit fee and the late fee of$100.00 are not received by 12:OOPM on
Friday, February 14, 2020.
