54 Felt Street Notice of PSS 11-8-19 RECEIVED CLEAT' NOV 1 3 2019 Clean Soils Environmental,Ltd. 33 Estes Street Sf�®jT c CITY OF SALEM Ipswich,MA 01938 1 L►J BOARD OF HEALTH 978-356-1177 ENVIRONMENTAL cleansoils.com November 8, 2019 City of Salem Mayor Kimberley Driscoll David Greenbaum, RS BOH 93 Washington Street Salem, MA 01970 Subject: Public Notification for a Permanent Solution Statement Homeowner #2 Fuel Oil Release to Soil Only 54 Felt Street Salem, MA MassDEP Release Tracking Number 3-35852 CSE Project No.: 2019.66 Dear City Officials: Clean Soils Environmental, Ltd., (CSE) is hereby notifying you, in accordance with the Massachusetts Contingency Plan (MCP), 310 CMR 40.1403, that a Permanent Solution with No Conditions (PSNC) Statement has been submitted to the Massachusetts Department of Environmental Protection(MassDEP) regarding the property referenced above; the conclusions of this Permanent Solution Statement are as follows: On September 13,2019 at approximately 1:53 PM, a homeowner reported a MassDEP 72-Hours release related to the removal on 275-gallon underground storage tank(UST). The affected soil at the Disposal Site was measured to extend approximately: 11 ft. long, up to 4 ft.wide, and down to 9 ft. below grade. For the purposes of this Permanent Solution Statement,the Disposal Site is considered to be the boundaries as shown on Figure 2. Remedial activities including the excavation,removal,and recycling of the localized approximately 7 tons of virgin petroleum contamination soil. The post-remedial soil testing results indicates that residual amounts of EPH fractions and target PAH analytes remain in the soil at the Site below the applicable MCP Method 1 Soil Standards, and are consistent with approaching background conditions at the Property. Due to the removal actions, the limited extent of residual soil contamination, groundwater not being encountered during excavation activities,and the distance of the Site from the nearest occupied building and surface water body,it is highly unlikely that groundwater, indoor air,surface water,or sediment have been significantly affected by the release. The goals of the IRA have been met through the remedial activities which has addressed the time-critical release and accomplished stabilization/control of site conditions.In accordance with 310 CMR 40.0427(3), an IRA Completion Report is not required for this Site because a Permanent Solution Statement is being submitted to MassDEP within 120 days of first informing MassDEP of the need to conduct an IRA at the Site. The results of the Method 1 Risk Characterization indicate that a condition of"No Significant Risk"of harm to health, public welfare,and the environment has been achieved for both current and future conditions. No further response actions are required to achieve a condition of No Significant Risk for any current or future foreseeable Site uses. This determination is not predicated on any activity and/or use limitation (AUL), grant of environmental restriction (GER), or other restriction on potential future activities and/or uses at the release location. It is CSE's professional opinion that the requirements for a Permanent Solution Statement with No Conditions specified in 310 CMR 40.1000 have been met for RTN 3-35852.
