Dear Mr. Thibodeau:
On behalf of Motiva_ EnterprisesLLC(Motiva); Sovereign Consulting Inc. (Sovereign) is
requesting a Road Opening Permit to install three (3) monitoring wells along Silver Street as
depicted on Figures l through 3. Motiva is conducting an environmental investigation for a
release of gasoline that was discovered at the Shell Service Station located at 146 Boston Street
during lune 2004 and subsequently assigned Release Tracking Number 3-23596 by the
Massachusetts Department of Environmental Protection. Part of this investigation entails the
installation of monitoring wells to delineate the nature and extent of the release pursuant to 310
CMR 40-0000 of the Massachusetts Contingency Plan.
At this time, Sovereign anticipates these wells will be installed over the course of two days. Dig
Safe and all other applicable utility companies will be notified at least 72 hours prior to drilling.
The first day of work will entail clearing each boring of utilities to 8 to 10 feet below grade using
vacuum excavation and air knifing. Once cleared, each 10 -inch diameter borehole will be
temporarily backfilled with excavated soils to grade and compacted. The second day of work
will consist of advancing each borehole to approximately 35 feet below grade using a hollow -
stem auger drill rig. Once at depth, a 2 -inch diameter PVC well will be installed in each location
and completed at grade with an 8 -inch diameter traffic -rated roadbox secured in a 2 foot by 2
foot concrete pad as depicted on Figure 4. The surrounding work area will be completely swept
and all excess soil cuttings will be drummed, transported, and stored at the referenced Shell
Service Station pending appropriate disposal.
Geosearch, Inc. of Fitchburg, Massachusetts will be the drilling contractor installing these wells
under the direction of a Sovereign Engineer or Scientist. A copy of their Drain Layers License is
provided as Attachment 1.
