Our office has received your letter dated 9-9-2019 with respect to the violation with
respect to the garage on this property. We have just been assigned the property after the
completion of the foreclosure sale. Currently, as of this letter we are working to address the
preservation items.
The property is scheduled to get trashed out,landscaping addressed and then we will be
able to obtain bids for ` full repairs' and send them to the bank for review. We will schedule the
exterminator as soon as the trash out is completed.

Dear Property Owner:
This office has received many complaints regarding rodents, raccoons and other animals
entering your property located at 354 Jefferson Avenue in the City of Salem,
Massachusetts.
An on-site inspection was conducted on July 30,2019 by David Greenbaum,Health
Agent for the Board of Health. At the time of inspection,it was noted that the garage
roof has collapsed and.there are potential entry points into the house allowing for the
entrance of rodents, skunks and other animals. Additionally,there is trash piled up in the
drive way in front of the garage and there are areas of the yard that are overgrown. These
conditions create an ideal harborage for rodents.
Raccoons,rodents,etc.are considered a neighborhood nuisance and community health
hazard.
The garage roof must either be repaired in accordance with all applicable codes and
regulations and all appropriated permits must be obtained. The overgrowth must be cut
back, and all trash must be removed and properly disposed of. The property, including
the house and garage must be exterminated by a licensed exterminator. After
extermination has taken place the property must be monitored to ensure that fature rodent
activity does not occur.
In accor&nee with Mass General Laws,Chapter III,Section 123 you are ordered to
retain the services of a licensed pest control operator and suppress any rodent or
animal population on this property within fourteen(14)days of receipt of this order
and a copy of this invoice must be forwarded to this department upon completion of
the extermination.
Should you be aggrieved by this order,you have the right to request a hearing before the
Board of Health. A request for said hearing must be received in writing in the office of
the Board of Health within 7 Days of receipt of this order. At said hearing,you will be
given an opportunity to be heard and to present witness and documentary evidence as to
why this order should be modified or withdrawn. You may be represented by an
attorney. Please also be informed that you have the right to inspect and obtain copies of
all relevant inspection or investigation reports,orders,and other documentary
information in the possession of this Board,and that any adverse party has the right to be
present at the hearing.
