OBSERVATIONS:
The undersigned arrived on site, as requested, to observe the placement of 10 cubic yards of 3000 psi
concrete for the pier footings. Please see attached sketch for approximate locations.
Prior to the placement of concrete, reinforcing steel was checked using the approved construction
drawings approved by the structural engineer of record. The correct number of the properly-sized bars
appeared to be present, and they appeared to be positioned as specified within acceptable tolerances.
Reinforcing bars appeared to be properly supported and appeared to be tied so that concrete placement
should not cause displacement.
The required concrete test limits were determined to be a slump of inches, a batch to placement time of
90 minutes or less, and concrete temperature range of 55 to 90 degrees Fahrenheit. Concrete observed on
this date appeared to be placed in compliance with project specifications with regard to temperature
and batch to placement time.
The following samples were obtained for compressive strength testing in our laboratory:
Set A: 1 set of 8 lab cylinders (3000 psi) for pier footings in Building 2.

Observation Summary: A site visit was made to the above referenced project to observe earthwork
construction:
The undersigned arrived on site, as requested, to observe the compaction of backfill soils for the
building slab. Site earthwork was performed by Pitt Pipeline Excavating company. Please see the
attached sketch for approximate locations of tests and areas observed.
Placement of an offsite recycled concrete backfill soil from Caruso was placed in approximately 8 inch
lifts. The Sand Cone test method (ASTM D-1556) was used to check the compaction of the backfill soils.
Test results indicated that the compacted material, at the areas and elevations tested, met or exceeded
the project requirements of the maximum dry density as obtained in our laboratory.
Areas tested were based on stakeouts or work limits provided by others and were not independently
confirmed by UTS.
