Dear Owner,
This Department was called to your property by the Salem Police due to concerns about the roof being torn off.
When I arrived at the property,the entire front half of the rubber membrane roof was flapping in the wind. The
membrane was up against the cupola which prevented further tear off of the roof.. After investigating the attic
area and second floor ceiling, I determined that rain water was penetrating the building causing an unsafe
Electrical condition. At this point I shut down the Electrical mains and officially removed the Occupancy for the
building.
To re occupy the building,the roof must be repaired or replaced by a licensed individual. Additionally, the
Electricity must be checked out by a licensed Electrician before being turned back on. If you have any
questions ,please contact me directly.

Dear Owners,
This Department,removed the Occupancy temporarily from your building on 10/29/12.When we last spoke
you were expecting the roofer to begin repairs the next day. At that time I stated that the roof needed to be
repaired(at least temporarily) and that an electrician had to inspect the wires to allow the Occupancy permit to
be re-issued.
. To date, it appears this has not happened.Under the Authority of the Mass State Building Code 780 CMR
section 101.4.4,you are required to maintain all systems in your building in a safe and operable manner.Failure
to complete this work will result in Municipal Code rickets and further enforcement actions.If you feel you are
aggrieved by this order,you appeal is to the Board of Buildings,Regulations and Standards in Boston.

your daughter stating the fact that all the violations
sent to you on January 11 , 1996 were completed and ready
for inspection. I conducted an inspection at 11 : 00 a .m.
on January 16 , 1996 in the following violations have not
been corrected:
1 . Patch all plastered walls and ceilings in apartment .
( The bathroom ceilings has not been touched) .
2 . Toilet not working correctly.
3 . Combination window in hallway still needs repairs .
4 . One basement window still needs to be repaired .
5 . Door to the outside from basement must be made weather
tight .
6 . Electrical wiring has to be completed.
7 . Water heater must have new duct work per gas company.
8 . Windows in bedroom must be repaired to open properly.
(These windows are a means of egress in case of fire ) .
9 . Repair siding at exterior of building at corners .
10 . Master bedroom window has a major leak.
11 . Replace boards that are rotted and unsafe at front
porch.
12 . Replace broken glass at front door.
13 . Weather strip front door and exterior combination
door .
