An on site inspection conducted by Virginia Moustakis, Sanitarian of the Salem Health Department on February 17, 1993 noted a large accumulation of discarded building materials at the back of the property and on the porches. The access to and from the building has been obstructed because of this material. The above are in violation of State Sanitary Code, Chapter II, 104 CMR 410.000 and are also creating potential fire, health and safety hazards for the occupants and area residents and the general public. You .are hereby ORDERED to take 'immediate corrective action to remove said materials within 7 DAYS of receipt of this order and properly dispose of the same in an approved manner. Kindly contact the Building Department at 745-9595 ext. 381 and Fire Prevention at 745-7777 regarding permits and the usage of a dumpster. Failure on your part to comply within the specified time will result in a complaint being sought against you in Salem District Court. Should you be aggrieved by this Order, you have the right to .request a hearing before the Board of Health. No commercial establishment shall place or cause to be placed more than four barrels or other containers of ordinary commercial wastes or any extraordinary commercial or industrial wastes or tree waste upon any sidewalk or way for disposal. At that time you expressed a desire to have your own painters give an estimate to complete the punch list. We agreed with this and requested that we have another painter also quote on this punch list. On July 30,2004 on our request you were contacted by George Glidden of Glidden Painting Co. George asked to come to your home and do an estimate. Mr.Glidden was told he was not welcome to wine and do the estimate. This is perplexing for us to understand as we are trying to complete our punch list and obligation to you. I hope we can work together and resolve this matter as soon as possible. The service included livening up the new wiring in our attic build out project. As I understand it Tom was waiting for John the inspector that had done the rough inspection(common practice as I understand. I was upset to get a phone call from Wendy saying,"Tom is fired and we don't want him on our property anymore". Tom Mctigue is the licensed electrician of record on your project. You are required to allow him to have his work in- spected by the City of Salem Electric Department. I am also disturbed from a safety and liability point of view that you turned on the new circuits for the attic before they were inspected. We deliberately told you not to do this before inspeo- tion. We are going to insist that the Electrical inspector inspects the project immediately to protect you and everyone involved. I am very disappointed that our relationship in this project has gotten to this level. I once again ask for your co-operation to help us complete a very few items so you can enjoy your new space.
