FIRE DEPARTMENT CERTIFICATE OF APPROVAL FOR BUILDING PERMIT
In compliance with the provision of Section 113.5 of the Massachusetts
State Building Code, and under guidelines agreed upon by the Salem Bldg.
Inspector and the Salem Fire Chief, the applicant for a building permit
shall obtain the Certificate of Approval (see reverse side) and stamped :--
plan approval from the Salem Fire Prevention Bureau.
Said application and approval is required before a building permit may be m
issued. The Massachusetts State Building Code requires compliance z 3
approval of the Salem Fire Department, with reference to provisions of G: m w
Articles 4 and 12 of the Building Code, the Salem Fire Code, Massachusetts �- p I
General Laws, and 527 Code of Massachusetts Regulations. CO

The applicant shall submit this application with three (3) sets of plans, g
drawn in sufficient clarity, to obtain stamped. approval of the Salem Fire �- ►- ¢ E
Department'. This applies for all new construction, substantial o w w G
_alterations, change of use and/or occupancy, and any other approvals a w <
required by the Massachusetts General Laws,- and the Salem Fire Code. a z a
Exceptions Plans will not be required for structural work when the
proposed work to be performed under the building permit will
not, in the opinion of the Building Inspector, require a
plan to show the nature and character of the work to be
performed. —
Notice: Plans are normally required for fire suppression systems,
fire alarm systems, tank installations, and Fire Code
requirements.
Under the provisions of Article 22 of' the Massachusetts. State Building
Code, certain proposed projects may not require submission of plans or
complete compliance with new construction requirements. In these
cases, provisions of Article 22, Appendix T,. and Tables applicable
shall apply. This section shall not, however, supersede the
provisions outlined in the Salem Fire Prevention Regulations, Chapter
1489 MGL, or 527 Code of Massachusetts Regulations. All permits for
fire code use and/or occupancy shall apply for the entire structure;

1. #12,14,16 Weatherly Drive (3 T-1 Town Houses) basement slabs are being
poured in stages with construction joints. Building T-12 has vapor barrier
and welded wire mesh in place. All three town houses are framed and
sheathed. Tyvec is being installed around window jambs prior to
installation of windows. Floor joists have all received bridging at
midpoints. Interior partitions are installed. Exterior stone wall is being
finished in the rear of building #12. Grace Ice Shield is in place over bay
windows. The copper installer was measuring for fabricating bay window
roofs.
2. Site work: Storm sewer is continued to the rear of 32 America Way with
catch basin.
3. #32 America Way: Basement partitions are in place over finished basement
slab. First floor framing is complete including sheathing. Foundations are
being poured today.
8. #34 America Way: Unit #2 passed papers today and will be occupied tomorrow.
Unit 34-1 signed a Purchase & Sales agreement. A concrete walk was
installed from Unit 34-1 to the garage.

1. 12, 14, 16 Weatherly Drive: 75% of the clapboarding is done.
All windows, doors and skylights are in place. Roofs are shingled. The
garage to unit 16 is framed and shingled. Rough wiring and Plumbing has
been started. HVAC Ducting is complete. Thermal caulking has not yet
been started. Copper crickets and bay window roofs are installed. Rear
chimneys are completed.
2. 32 America Way: Roof is framed. Chimneys are started. Exterior soffets
and trim will be started tomorrow. Concrete pad and patios are being
smooth finished. Footings with key ways are in place for the two car
garage.
3. Site: Bituminous paving binder is in place to the lower level garages.
Form work contractor will erect walls for garage and front entry of 32
America Way tomorrow. All catch basins and storm drains are complete in
the rear of #32 Amer. Way. All electrical conduit is sleeved into
basement foundation wall with pull strings.
