The public hearing was closed on February 5, 1998. At a regularly scheduled meeting of the Planning Board held on February 5, 1998, the Board voted by a vote of seven (7) in favor, none (0) opposed, to the application as complying with the requirements of Site Plan Review and the Wetlands and Flood Hazard District, subject to the following conditions: I. Conformance with Plan Work shall conform to plan entitled "Carol Way Condominiums, Loring Avenue, Salem, MA " prepared by VHB, 101 Walnut Street, Watertown, MA 02272, dated March 25, 1997, revised through December 8, 1997, containing sixteen(16) sheets. (Site Plan) 2. Amendments Any amendments to the above referenced Site Plan or waiver of conditions contained herein shall require the approval of the Planning Board. 3. Maintenance a. The applicant shall employ an acceptable method or means for the holding and disposal of trash after site development with a copy of this method sent to the Planning Department and Health Department in writing for their approval. b. Road, storm drainage system, ground maintenance, and snow removal shall be the responsibility of the applicant, his successors or assigns. C. Winter snow in excess of the snow storage areas on the site shall be removed off site. 4. Utilities Utility installation shall be reviewed and approved by the City Engineer prior to the issuance of building permits. 5. Landscaping Landscaping shall be in accordance with plan entitled, "Carol Way Condominium, Salem, MA", sheet C-1 I of 16, prepared by VHB, dated March 24, 1997, revised through January, 1998. containing sheets 1-7. a. Maintenance of landscape vegetation shall be the responsibility of the applicant, his successors or assigns. b. After all project landscaping is completed, the applicant shall be required to add further landscaping if such is required by the City Planner. 6. Loring Towers Road Loring Towers Road shall be constructed and used as a means of egress for the residents of the two (2) rear buildings in Phase 4. Loring Towers Road shall be posted as a one-way street. The maintenance of the new section of Loring Towers Road shall be the responsibility of the developer, his successors or assigns. 7. Pond The Applicant shall submit a final layout and planting plan for the pond area to the City Planner. In addition, the applicant, his successors or assigns shall be responsible for the maintenance of the pond and the surrounding area. The applicant agrees to covenant that the maintenance of the pond will be a part of the Master Condominium Documents for Phase 4. 8. Board of Appeals All requirements of the Board of Appeal Decision dated December 17, 1997, shall be strictly adhered to. 9. Conservation Commission All requirements of the Order of Conditions issued by the Salem Conservation Commission shall be strictly adhered to. 10. Board of Health All work shall comply with the requirements of the Board of Health. 11. Fire Department All work shall comply with the requirements of the Salem Fire Department. 12. Building Inspector All work shall comply with the requirements of the City of Salem Building Inspector. 13. Director of Public Services All requirements of the Director of Public Services, as stated in his May 27, 1997 memo to Craig Wheeler (see attachment A), shall be strictly adhered to. 14. Clerk of Works A Clerk of Works shall be provided by the City and paid for by the applicant as deemed necessary by the City Planner. 15. Construction Practices All construction shall be carried out in accordance with the following conditions: a. No work shall continence before 8:00 A.M. on weekdays and on Saturdays. No work shall continue beyond 5:00 P.M. There shall be no work conducted on Sundays or holidays. Inside work of a quiet nature may be permitted at other times between the hours of 7:00 A.M.-9:00 P.M. Monday-Friday, excepting Saturdays and all day Sunday. b. All reasonable action shall be taken to minimize the negative effects of construction on abutters. Advance notice shall be provided to all abutters in writing at least 72 hours prior to commencement of construction. C. All construction vehicles shall be cleaned prior to leaving the site so that they do not leave dirt and/or debris on roadways as they leave the site. d. The Applicant shall provide portable restroom facilities on site during the entire construction phase. e. All construction shall be performed in accordance with the rules and regulations of the Planning Board, and in accordance with any and all rules, regulations and ordinances. 16. Violation Violation of any condition of this decision may result in revocation of this permit by the Planning Board in its sole discretion. Vinnin Assoc.Realty Trust Carol Way,Weatherly Dr. , America Way. VHB Inc. , Watertown MA. We have completed our review of the above referenced project and offer the following comments . 1 . All buildings that have garages must have an "MDC" separator on the drain lines . 2 . Sheet C-8 must show information on the proposed pump station as well as the size of the forcemain. 3 . Sheet C-14 must designate all hydrants as American Darling B62B, "open right" . 4 . Minimum water pipe size of 8" to hydrants ( laterals can be 6" if desired) . Water line should be looped from building "D" to Weatherly Dr. and should be 8" . This office participated in hydrant testing conducted on May 12, 1997 . The results of the tests are included in a report dated May 23, 1997, prepared by Tata & Howard Inc. , Westborough MA. The results of the test indicate there sufficient quantity and pressure to support the additional development proposed in phase 4 of this project. There has been some concern voiced by residents about low water pressure . Although we did not find this to be the case at the hydrant, it may be the case on the upper floors of existing buildings. This may be caused by water saving showerheads and toilets . FIRE PUMPS ARE THE PURVIEW OF THE FIRE DEPARTMENT AND ARE NOT DISCUSSED IN THIS MEMO. 5 . The entire length of existing sewers ( from building "D" to the 16 inch line) were cleaned and inspected using closed circuit T.V. The results are contained in 9 video tapes prepared by T. Mackey & Sons . There is adequate capacity in the existing system to accommodate flows from the addition of Phase 4 flows . We have identified from the tapes a few areas that need to be repaired. These are minor in nature and should be repaired prior to the addition of any new flows. We recommend that the developer contact this office prior to making repairs. 6 . This office did not review traffic conditions . As shown in items 4 and 5, our staff and others spent considerable time and effort reviewing both existing and proposed conditions of both the water and sewer systems in the area of the project. If the above recommendations are followed and the project is constructed as shown on the drawings, the impact of the additional units should not be detrimental to either the water or sewer systems .
