Dear Mr. Maybury:
On Thursday, February 4, 1999, a reinspection of your property located at 284
Washington Street was conducted with the Salem Health Department and Mr. Carl
Ducey. As a result of this inspection the following building violations were identified and
need to be corrected immediately.
A. Exterior of building/common areas.
1. Front steps missing handrail.
2. No exterior light for front steps.
3. No door bells for apartment units.
4. Occupants names not posted.
5. Trim molding around second floor window missing.
6. Door closers missing on exterior doors.
7. Fire alarm zones not identified.
8. Rear porch railing damaged.
9. Equipment being stored in front entrance.
10. Lock on second floor porch door missing.
11. Illegal storage in rear of building.
12. Illegal furnace on exterior of building.
B. Apartment#2 left.
1. Bathroom exhaust fan improperly installed.
2. No GFI outlet in bathroom.
3. Light fixture in bathroom wired wrong.
4. Repair hardwired smoke detector in middle room.
5. Installed hardwired smoke detector in front bedroom.
6. Light fixture above kitchen sink wired wrong.
7. Holes in kitchen wall.
C. First floor shop/basement.
1. No emergency exit sign or lighting in shop.
2. No smoke detector in shop area.
3. Open and exposed electrical boxes.
4. Combustible material to close to furnace.
5. Electrical panels obstructed and not accessible.
6. Gas and water service not accessible.
7. Electrical service ground to water meter missing.
8. No handrail on basement stairs.
9. Cover missing on front of gas furnace.
10. Extension cord used on furnace vent.
D. Apartment#2 right.
1. No access needs to be inspected.
You are hereby ordered to correct these violations immediately and to schedule a
reinspection of the property before February 24, 1999. Failure to do so will result in legal
action being taken against you in Salem District Court.

units, it was noted that the water piping was installed with less
than the required depth, required by law ( to cause frau freezing ) .
Tempering device used for controlling hot water was not 100%
controllable, identification tag was not on tankless.

At its September 20, 1990 monthly meeting, the Construction Materials Safety Board (CMSB) completed
review of Taylor Manufacturing, Ine.'s application for approval of Taylor OUTSIDE WOOD FIRED HOT
WATER FURNACES.
The CMSB has spent considerable time in its review of these products largdly based on the differing
classification of this equipment as either furnaces,as Taylor has defined them,or atmospheric boilers (based
on the actual design of these devices, the CMSB categorizes these units as atmospheric boilers and not
thouof thrh the CMSB recognl= that the use of rtmem with fan blowers
en atmospheric boilers would emulate warm air operation).

In light of the Board of Boiler Rules conclusions regarding the wood burning products under consideration,
the following motion was voted at the CMSB September meeting:
On MOTION of F. Harvey and SECONDED by C. Theodore, it was unanimously voted to inform
Taylor Manufacturing, Inc. of Elizabethtown,N.C. that relative to the Massachusetts State Building
Code(780 CMR - the Code),Taylor'Outdoor Wood-Fired, Hot Water Stoves', models of 280, 450,
75o, 1000, 2000, 3000, 4000, 6000 and 10,000 gallons, do not require special approval and may be
installed in the Commonwealth subject to conformance with all other applicable State and Federal
regulations as well as any requirements of the Manufacturer. ,
&*neral points of clarity include the necessity to ensure these products, if installed in close proximity
to combustible construction,are situated sufficiently away from such construction or such combustible
construction is otherwise protected from the potential conduction, convection or radiant heating
impact of this equipment (even though the normal installation of such equipment is intended to be
'outdoors', if such equipment were either placed outdoors but close to a combustible building %all
or if such equipment were eventually enclosed in an auxiliary building or other structure, care must
be taken to prevent ignition of combustible Construction).Normal recommended clearances for heat•
producing appliances may be found in Tables 21101 and 2110.2 of Article 2110 of the Fourth Edition
of the Code or Table 2512.1.1 of the Fifth Edition of the Code in conjunction with Table M•1101.2
of Article 11 of the 1987 BOCA Mechanical Code•Note that ultimately,Taylor,as the Manufacturer
of the subject equipment, should determine that such clearances are satisfactory.
The conclusions of the CMSB regarding the subject Taylor products are based on the application materials
dated 812.5/89, 921/89 and the associated supporting materials provided. Any change in name, design,
performance,installationor intended use will require Taylor Manufacturing,Inc,to submit such products again
to the State Board of Building Regulations and Standards for consideration of approval.
Taylor Manufacturing, Inc. Is advised that the CMSB does not evaluate product efficiency or the advisability
of using one product as opposed to any other and therefore Taylor Is requested not to utilize this letter
response in any advertising or promotion of said products,

and steam rate for such equipment and when utilized in non-residential applications•for such equipment sizes
