Dear Sir/Madam:
- Based on an inspection of the above referenced property, the
following has been found:
The rear porches need repairs or reconstruction. Porches should
not be used until such repairs or reconstruction have been made.
Although I was not invited into the first floor apartments, I was
informed by Mrs. Englehart and the Health Agent that only one way in
and out is available. This is a violation of the Massachusetts State
Building Code and must be corrected immediately.
Please contact this office as to the above violations and your
intentions.

General Laws, 105 CIUR 400.000: State Sanitary Code, Chapter 1: General
Administrative Procedures and 105 CMR 410.000: State Sanitary Code, Chapter
II: Minimum Standards of Fitness for Human Habitation, an inspection was
made of your property at270 Washington St. , Apt. 5 Salem, Massachusetts,
occupied by Common Areas This inspection was
conducted by V. Moustakis/D: Englehart, Agent Salem Health Department, on
8/20/86 at 9:30 A.M. for Mrs. Shinnick
Based upon said inspection, you are hereby ordered to take the following
action within 24 hours of receipt of this order:
Must repair the support columns which are pulling away from outer
carrying beams on the back porches-both (2nd and 3rd floor) porches
must be cleaned and doors leading to porches padlocked. - Contact
Building Inspector immediately porches to be padlocked until repairs
are made or porches removed.
Front and back hallways have refrigerators , bicycles, windows, rubbish,
garbage and other debris and glass and must be cleaned at once.
Must provide a coverplate for the electrical outlet in the back hallway
1st floor.
Name of owner or agent address and phone number is not posted in front
hall 1st floor - Must post on sign at least 20 inches square in size.
Labels should be put on the light switches in the back hall so the
fuel switches will not be used in error.
Must provide light bulbs and operating switches in the front and back
halls.

Based upon said inspection, you are hereby ordered to take the following
action within 5 days of receipt of this order:
Must provide hardwired smoke detectors in both front and back hallways
of this (5) apartment building - Contact Fire Department as to quantity,
type and location of detectors.throughout building
XThe first floor apartments must have 2nd means of egress - Contact
Building Inspector at once.
Exterior of building has overgrowth of grass and weeds and wind blown
debris and glass that must be cut and removed.
Rubbish barrels (rodent-proof material b watertight) with tight-fitting
lids must be provided ins-sufficient quantity for all tenants (usually
(2) per apartment.
Must repair or replace broken screen/window pane in Tucker apartment.
Based upon said inspection, you are hereby ordered to take the following
action within 30 days of receipt of this order:
Must provide emergency lighting in both front and back hallways in this

Dear Mr. White:
I have left several messages on your answering machine at 1-617-
598-3314 and have failed to hear from you. I received a call from a
concerned citizen concerning a piece of metal facia and soffitt hanging
at roof level in rear of property located at 270 Washington Street . I
have made an inspection and have concluded that the work must be done
immediately to fasten loose metal. This could cause serious injury to
by passers if it should fall.
Please correct this problem immediately. Thank you in advance
for your anticipated cooperation in this matter.
