The licensed contractor was Edward McQueeney (CS 08554) Mc McQweeney undertook the permitted work and was in regular contact with this office. It should be noted that Mr. McQueeney;s scope of work was limited to the sub. floor lintel and wall repair and was not a permit for the complete renovations to the restaurant. On or about August I, 2003, Mr McQueeney came to my office to express concerns about other work being done by others and he did not wand to be held responsible for that work.I instructed Mr McQueeney that he would be responsible as a licensed contractor/ At that point Mr.Queeney advised me that he would like to withdraw his license from the job. I instructed him to provide this office with a letter indicating this and I received it August 6, 2003. Both in my previous dealings with Mr. McQueeney and the restaurant build out I have found I have found Mr. McQueeney to be honest and forthcoming. It should also be noted, I inspected the work completed by Mr. McQueeney after his withdrawal from the project. No problems were found. S This is an attempt to clarify the sequence of events. To the best of our knowledge, information and belief, the mechanical systems are in compliance with the code's for new construction. The design narrative for the space is as follows: Main Dining Room/Toilet Rooms/Corridor The Main Dining Room,Toilet Rooms and Corridor are heated and cooled by 2, Six Ton,air to air heat purnps.which re=circulate 100% of their air volume. Ventilation is achieved by natural means as allowed by Section 402 of The 1998 International Mechanical Code which requires 40 ft square of openable area to the outdoors(4%of 1,000 square feet). This report is based on our observations, qualifications, and information provided to us during this visit. It does not claim to be an itemization of all structural problems, and is intended only to provide the client with a general idea of the structural problems observed during the walk-through inspection. OBSERVATIONS AND RECOMMENDATIONS The purpose of this visit was to discuss the possibility of creating a ten-foot opening in the first floor, at a spot where the building was changing direction. According to our information, this beam was supporting only one floor plus a roof on one side of the building only. We calculated the beam to be approximately three 2x12 LVL or microlam supported, on not less than 4x6 plus 2x4 and we made a sketch indicating how we recommended this to be built. We then looked at the beam, which was damaged and there we recommended the addition of two 2x12 microlams or LVL, which would be cut in shape to exactly fit the support at each end. Note that all flush frame members must have hangers. Those calculations were made quickly on the site and doing so mistakes could be made in the execution of those recommendations, we, therefore, recommend that plans be made to properly detail all connections supports etc. �y It is also important that the Structural Engineer visit the site during construction, so as to verify its compliance with the plans and structural recommendations. 1 i ' This report addresses only those structural problems observed during the , walk-through and documented above. Since few structures were exposed during the visit, other structural problems may be concealed behind finishes, plaster ceilings, and walls. We did not implement computations or verify compliance with earthquake code regulations, and do not claim that the observed structural members are of the proper size and properly transmit the load from floor to floor. The Structural Engineeris riot responsible for determining the existence of insect infestation and waterproofing. This report and analysis is based upon observations of the visible and apparent condition of the building and its major components on the date of this inspection. We strongly suggest that the floor be exposed at the base and that this wall be properly addressed so as to transfer the load of all the floors above to the supporting bearing point in the crawlspace underneath. As it is not possible to observe the structure, where picture #6 was taken, we suggest that not less than.2 ft. of flooring be removed to properly expose the support of that wall and the foundations. At that time, we will be able to properly advise you on how to address this problem. This report is based on our observations, qualifications, and information provided to us during this visit. It does not claim to be an itemization of all structural problems, and is intended only to provide the client with a general idea of the structural problems observed during the walk-through inspection. OBSERVATIONS AND RECOMMENDATIONS The purpose of this visit was your intent to create two openings in what appears to be the bearing wall perpendicular to the street between two units, one opening being 6' 6" and the other 11'. On the first opening, which is 6' 6", you are intending to put three 2 x 12's and this would be proper. We recommend that you use a 6 x 6 or 3" pipe for each support and that the support be properly borne on the girder below, which is the carrying girder supporting the first floor. This girder needs to be checked whether the girder has the proper capacity to support these columns, as during the visit, we did not have access to this area. However, given the small opening, it appears to us that there is not too much of a problem. On the other hand, where you propose to cut an 11' opening, this is much more difficult to do anhe beam would have to be substantial, possibly a steel beam or a very large microlam properly designed to support the load above. If you intend to do that, we would have to give you the proper size for the beam, which would be more like 8 x 14 microlam with two 6 x 6 posts on each end or steel columns. Again, it is very important that these posts be properly bome by the basement 1 girders, which should have the proper capacity to support the reactions and this time the concern is more important as you may most likely need reinforcement or new footings. During our visit, we did not know the size nor the condition of the beam, which is supposed to carry the supports. -Nevertheless, we found—sorrie steel columns in the partitions and there .is a possibility there is a big girder or a big beam supporting the second floor. This is only a possibility, as we did not see the beam during our visit. Once it is exposed, we will be able to draw better conclusions and the beams may not be necessary. We also noticed that the floor was made of full 2 x 9-1/2 at 1' 4" on center spanning 18' 4". This is a small size, definitely not able to support a restaurant load, which would be 100# per sq. foot. We strongly recommend that possibly an intermediate support be introduced which could be made of beams and columns, properly sized and borne on good properly designed footings in order to cut the span in half. This would bring the proper capacity to the joists. Another Way would be to double up the joists with proper members to insure that the capacity would be reached. (The supporting beams will also need to b e verified for the 100 lbs. live and dead loads). Note that the wall of concern is most likely used as a shear wall and enough capacity must be left in this wall to resist the lateral loads. We recommend that after properly exposing the structures, structural plans be drawn, indicating the structures as they exist at the time of the investigation, and including all new reinforcements necessary to bring the structures up to Code. Such plans would be useful in that: • They would allow necessary permits to be obtained for the repair work. • They would provide a good way to estimate the work to be executed. • They would document the executed work for future reference, such as]n the . event of later alterations to the building. It is also important that the Structural Engineer visit the site during construction, so as to verify its compliance with the plans and structural recommendations. Conclusions in this report are based on the normal working life of various structural items. Predictions of life expectancy and the balance of useful life are necessarily based on industry and/or-atatistical comparisons. It is essential to understand that actual working conditions can alter the useful life of any item. Previous use or misuse, irregular maintenance, faulty manufacture, unfavorable conditions, unforeseen circumstances and acts of God can make it impossible to state precisely when a speck item would require replacement.
