maintained as required by this office and the National Fire Protection
Association . A copy of the minimum requirements are enclosed .
The services of this office have been overly extended in order to

On January 18, 1995 an inspection was done at Bleacher's, 143 Washington
Street. The following is a list of violations that must be corrected within
72 hours upon receipt of this letter:
1. Boiler room needs to be secured and all unrelated heating equipment
needs to be removed.
2. Basement room where electrical equipment is located must be cleaned
of material and emergency lights secured per Salem Fire Department.
3. Door to basement must be marked and locked.
4. Side egress door must be made to work, vestibule must be cleared of
debris (vacuum, broom , ect) .
5. Pool table area, there is duct covered under table. This must be
secured properly.
The following violations are all located in the kitchen area are:
1. Permit must be secured for water heater.
2. Back door made to open and fan removed.
3. Dumb waiter secured and sealed.
4. Cellar stairs repaired as well as wall.
5. Hoods cleaned properly.
6. Fix floor tiles correctly.
7. Back door to street needs to be fixed so they work independently.
8. Refrigerator in basement must have all three drains removed or
sealed.

The following violations were noted during an inspection conducted with the Salem Health
inspectors, on August 25, 1994, at 4:45/P.M. , and must be remedied immediately:
1- The removal and use of all and any electrical extension cords. Cords were found
wraped around the metal heating baseboards. Cords were found loose on the floor where
customers could trip on them.
2- Found numerous outlets and switch plates missing leaving wiring exposed.
3- Outlet on the wall near the rear pool table has wires exposed.
4- The emergency lights throughout the establishment are NOT operational , there is evidence
of leaking batteries in the unit.
5- Numerous missing ceiling tiles throughout the establishment.
6- Grill in the kitchen use for make-up air lo'cated 'on the ceiling, full of grease.
7- No ground fault outlets near the bar sinks.
8- Electrical outlet dripping with old beer residue. The beer bottle opener is located
above the cutlet.
9- Electrical panel cover in the front basement is missing.
10-Compressors used for the beer coolers are caked with drippings of beer. The openers
are located above the compressors.
11-Second floor exit door jammed unable to open.
12-Found an accumulation of old furniture, old wood, and many other combustibles throughout
the building. . . . .all and any debris is to be removed from the building;
After conferring on site with .Joanne Scott, Health Agent for the City of Salem, relative
to the safety issues and the conditions found therein and specifically items #4 and 11 in
my report, it was decided, for reason of public safety to close the establishment.

Dear Mr. Wong:
Due to canplaints received from the Salem Fire Department on September 27,
1990, regarding the above referenced property, an inspection was made.
Present at the inspection on October 9, 1990, were Michael Barrasso and
Clarence Wong. Below is a list of violations brought to your attention.
Mr. Barrasso camiented that the violations were simple to rectify and would
be corrected prior to our next scheduled inspection. The violations are as
follows:
First Floor
1) . Second exitways blocked
2) . Exit signs in rear bar roan are missing
3) . Panic hardware and double key locks in rear bar roan not corrected
4) . No one present to test emergency lights
5) . No fire extinguishers at proper locations

108.4 Credentials: The building official or his authorized
representative shall carry proper credentials of his respective
office for the purpose of inspecting any and all buildings,
structures and premises in the performance of his duties under
this code.
108.5 Inspections: The building official shall make all the re-
quired inspections or may accept reports of inspections from a
qualified registered professional engineer or architect or others
certified by the Commission, and all reports of such inspections
shall be in writing; or the building official may engage such
experts as he may deem necessary to report upon unusual tech-
nical issues that may arise.
10_.8.5.1 Inspection and certt- ;, specified use groups:
The building-official-shall'periodically inspect and certify build-
ings and structures or parts thereof in accordance with
Table 108. A building or structure shall not be occupied or
continue to be occupied without the posting of a valid certifi-
cate of inspection where required by Table 108. A certificate
of inspection as herein specified shall not be issued until an
inspection„is made certifying_that-the:building=or-.-structureror,
parts thereof complies with all the applicable-requirements of
lthfsrcode,f and`until-the fee'is paid as specified in Table 108.
Municipalities may waive only in their entirety the fees as speci-
fied in Table 108 for buildings and structures or parts thereof.

Exception: Municipalities may revise or modify, or waive in
part those fees for buildings and structures or parts thereof
owned by the municipality, county or political subdivision
thereof and for buildings and structures or parts thereof
used solely for religious purposes.
108.6 Administrative procedures: The building commissioner or
inspector of buildings shall have the authority to formulate ad-
ministrative procedures necessary to uniformly administer and ,
enforce this code provided that such procedures do not conflict
with the rules and regulations promulgated by the Commission.

Dear Mr. Barasso:
As the result of a complaint brought by Acting Deputy Chief Brophy of the
Salem Fire Department on September 27, 1990, an inspection was made this
date by Captain Turner and Acting Deputy Chief Brophy, and the following
conditions were found to exist:
(1) The means of egress leading to the right rear exit from the first
floor restaurant area was obstructed by furniture and fixtures as
to impede proper egress from the premises. (Life Safety Code
Section 5-1. 7.3)
(2) The sprinkler system appears to have been neglected with no
evidence of proper testing and maintenence as required by Section
504.3 of the Salem Fire Code. Sprinkler loading (an accumulation
of dust and grease) was evident on numerous sprinkler heads.
(3) Several electrical junction boxes, in areas accessible to the
public, were uncovered exposing live electrical wiring (Section
314.6, Salem Fire Code).
(4) The automatic fire alarm system has not been tested within the
required period. Automatic fire alarms shall be tested at least
twice per year (Section 503. 7, Salem Fire Code).
(5) Exit signs within the premises not maintained. All exit signs
shall be maintained in a clear and legible condition and shall be
clearly illuminated at all times that the building is occupied
(Section 604.1, Salem Fire Code).
(6) Portable fire extinguishers failed to be maintained. All
portable fire extinguishers shall be selected, distributed,
inspected, maintained, tested and recharged in accordancd with

NFiPA 10 listed in Appendix A and Section F-508.2 of the Salem
Fire Code (Section 508.1, Salem Fire Code).
(7) An accumulation of combustible materials was being stored in the
basement of the building. Storage shall not produce conditions
which in the opinion and judgement of the code official will tend
to create a nuisance or a hazard to the public health, safety or
welfare (Section 304. 1, Salem Fire Code).
(8) The smoke door at the top of the stairs leading from the rear of
the second floor dance area to the exit on the first floor lacked
a self-closing device. All self-closing and automatic doors
serving as a means of egress or providing a fire or smoke barrier
as required by the State Building Code shall be maintained in
operable condition at all times in accordance with NFiPA 80 whrer
provisions of Article 6 of the Salem Fire Code do not
specifically cover conditions and operations (Section 605.1,
Salem Fire Code).
(9) Decorative material used on the second floor at the top of the
stairway is flammable. Materials used for decorative purposes or
display shall conform to 527 Code of Massachusetts Regulations
21.00 (Section 306.1, Salem Fire Code).
(10) APPEAL: Any person shall have the right of appeal to the State
Fire Marshal, from a decition of the Head of the Fire Department
or his designee. An application for appeal shall be based on a
claim that the true intent of the Fire Code or the rules legally
adopted have been incorrectly interpreted, the provisions of the
Fire Code do not fully apply, or an equally good or better method
of fire prevention is used. The application for appeal shall be
filed within ten (10) days after this notice was served.

Street. The following is a list of violations that must be corrected within
72 hours upon receipt of this letter:
1. Boiler room needs to be secured and all unrelated heating equipment
needs to be removed.
2. Basement room where electrical equipment is located must be cleaned
of material and emergency lights secured per Salem Fire Department.
3. Door to basement must be marked and locked.
4. Side egress door must be made to work, vestibule must be cleared of
debris (vacuum, broom , ect) .
5. Pool table area, there is duct covered under table. This must be
secured properly.
The following violations are all located in the kitchen area are:
1. Permit must be secured for water heater.
2. Back door made to open and fan removed.
3. Dumb waiter secured and sealed.
4. Cellar stairs repaired as well as wall.
5. Hoods cleaned properly.
6. Fix floor tiles correctly.
7. Back door to street needs to be fixed so they work independently.
8. Refrigerator in basement must have all three drains removed or
sealed.
