Dear Mr. Alex:
As per our April 11, 1990 office meeting regarding the proposed "Derby Square
Restaurant and Pizza", the Health Department makes the following recommendations
in accordance with 105 CMR 590.000; State Sanitary Code, Chapter X "Minimum
Sttindards for Food Establishments":
- Walls, floors and ceilings in food preparation, food storage ware-
washing areas, and all walk-in refrigerators, dressing rooms, toilet
rooms shall be constructed of smooth non-absorbent easily cleanable
surfaces. An example is stainless steel, "glassboard" or equivalent
material. -Zl 11
- You stated that the floor is to be covered with quarry tile and
serviced by floor drains.
- All equipment and utensils to be constructed of stainless steel or
an equivalent smooth easily cleanable material. Equipment to be
installed with adequate space surrounding it to allow for regular,
effective cleaning.
- As discussed, relocate the wall-hung handwashing sink with liquid
soap and paper towel dispensers or install a second hand sink for
effective use by employee in both food preparation areas.
- A three compartment sink to be installed for warewashing. A drain-
board to be installed at end of sink to allow for air drying.
- In new establishments at least one (1) service sink or curbed cleaning
facility with a floor drain shall be provided and used for the cleaning
of mops or similar cleaning tools and for the disposal of mopwater.
Handwashing or warewashing facilities or food preparation sinks shall
not be used for this purpose under any circumstances. '

As discussed, a commercial grade dishwasher is to be installed. If dishwasher
is not provided then, a product washing sink must be installed.
- The installation of exposed horizontal utility service lines and pipes
on the floor is prohibited. Exposed utility service lines and pipes
shall be installed in a way that does not obstruct or prevent cleaning
of the walls, floors and ceilings.
- All construction must meet the code requirements set forth by the Building,
Fire, and Electrical Departments as well as the Health Department.
- Any changes of this proposed food establishment requires Board of Health
approval.
The Health department appreciates your cooperation in this matter.
Contact this office to arrange compliance inspections throughout the construction
process.

Sirs:
As a result of an inservice inspection by Engine Co. #l., on the
above date, the following items were noted as violations of law,
and such action as the law requires shall be taken to correct same
wi, hin seven (7) days of this notice.
1. Excessive accumulation of combustibles in the basement of this
occupancy. An aisle of at least fourty-four (44) inches shall
be maintained for the entire length of the basement.
2. All other combustibles or materials which arestored within the
basement shall be stored in a neat manner, so as not to create
a hazard to life and property.
3. It was found that several electrical wires were found cut and
exposed, which are creating an extreme fire hazard. Several
junction boxes were without plate covers and exposed wires.
4. Evidence of a poor installation of the gas boiler flue pipe
which was shoved up into the vertical brick chimney exists and
soot is visible over the boxes underneath the connection. This
apparently shows that the metal flue pipe is not properly installed
into the vertical chimney and creates a fire hazard. This pipe
shall be repaired and the connection properly sealed up.
5. Emergency lighting was checked and the units .were found inoperable.
A licensed electrican shall be contacted to reactivate all emergency
lighting units.
6. Stairway door on third floor was found trigged open. All stairway
S doors must remain closed to prevent an open shaft for fire.

7. The fire alarm system was found inoperative and out of service.
The key was found left in the fire alarm panel, leaving the system
subject to vandalism. Said fire alarm system shall be serviced
by a licensed electrician and placed back in service.
