permit for the above project from. the City of Salute, The only
iters holding tip the issuing of this permit, is the glass Lobby
windows on the Washington street side of than building. This
glass, which you approved, is contrary to the code (section
856. 37) 0 and must be replaced with full, tezpered glass before
than permit will be issued.
Since your office has been aware of this problem and
has taken no action, to mate, we intend to hire a glass company
to do the work and bachcharge your firm, accordingly, unless you
inform us that you intend to perform your contractural responsi-
bilities in this matter.

This is in 'reply to your correspondence to Mr. Marion,
Clerk of the Works, of September 14 , 1977, with respect to
the certificate of occupancy for this project.
1. Lobby window glass--This will be replaced to
conform with the code.
2 . Sub-basement ventilation. This will be provided
in accordance with your meeting with our Mr. Ali
on September 21, 1977.
3. Custodial Room Ventilation-This will be provided
as per the above mentioned meeting.
4 . Egress Door lock, Stair #1-We are in the process
of reviewing this item with the building code
commission. We will comply with their ruling.
5. Detention area-Exit sign and emergency egress will
be provided as per discussions with our Mr. Ru11is
on -September 21,' 1977. An additional two exit
signs will be provided at Stair #2 .

Pursuant to our telephone conversation of today, with
reference to your letter of March 14 , to Andy Marion, we
will instruct the Contractor by copy of this letter, to
proceed as follows:
1. Mortar joints in the elevation shaft wall will be
filled from elevatorll platform when it is in place.
2.. Shell of block. Block behind toilet accessories will
be filled with over 2 of concrete to provide a solid
wall-minimum of 4" thick.
3. Fire dampers will be included in the ductwork at
louvers over doors at three story waiting room. We
understand that no damper will be required in opening
between elevator shaft and elevator machine room.

Dear My. Marion:
During our inspection on March 10, 1977 we noted that the reinforced
masonry block wall on the east wall of the elevator shaft had been
cut away leaving only one shell of the block. This wall thus loses
the requited two hour fire rating in the basement and second floors .
It was also noted that the mortar joints in the elevator shaft wall
are insufficiently filled to be acceptable for a rated wall.
There is no indication of any fire dampers , either being installed
or called for on the drawings , where ducts penetrate the rated wall
surrounding the three story waiting room.
Would you kindly give these matters your earliest attention so as
not to delay the issuance of a Certificate of Occupancy when that time
arrives.

After an inspection of the above premises, on September 15, 1977,
the following violations were sited by the Architectural Barriers
Board:
1. No curb cuts on Washington Street, at Church Street the curb cuts
are too steep, (not 1: 12)
2. Bathrooms : coat hooks should be 54 inches , grab bars should be
30 inches, there are no grab bars at the rear of the water closet ,
water closet should be 18 inches from the nearest wall, water closet
should be 18 inches from the floor to the top of the seat , the
mirror should be 38 inches from the floor to the bottom of the mirror ,
3. The handrails on the steps are a a inch too wide.
4 . The left side of the telephone, there is an obstruction to handi-
capped persons in wheelchairs ,
5. The exit threshold onthe north side of the building, facing the
parking lot , it too high and there are no 18 inch projections on
handrails on the stairs at same exit .
The Board requests a written notice as to your intent to comply , within
two weeks .

Pursumit to our inspection of September 99 1977, we note the following items which
should be attended to prior to issuance of a certificate of occupancy:

duced into the air handling units, particularly as we can find no evidence

3. The custodial room in the sub-basement shall not be occupied until
ventilation is provided for that room. Section 503.0 of the building
code is applicable. .
4. It is our opinion that the type of lock used on door 00.01 in the sub-
basement Stair No. 1, does not beet tha State Code. Barring a ruling
from the Tui=ding Coda Commission, *••7e ask that the lock be removed from
t"a egress side of any door which is in the path of egress.

Dear Mr. Marion:
During our inspection on March 10, 1977 we noted that the reinforced
masonry block wall on the east wall of the elevator shaft had been
cut away leaving only one shell of the block. This wall thus loses
the required two hour fire rating in the basement and second floors.
It was also noted that the mortar joints in the elevator shaft wall
are =insufficiently filled to be acceptable for a rated wall .
There is no indication of any fire dampers , either being installed
or called for on the drawings , where ducts penetrate the .'rated walL
surrounding the three story waiting room.
Would you kindly give these matters your earliest attention so as
not to delay the issuance of a Cert=ificate of Occupancy when that time
arrives .

As a result of an inspection conducted with Salem Building Inspector
Richard McIntosh, of the cell area located on the lower level of the
First District Court Building, 65 Washington Street, Salem, Mass. ; it has
been determined that public safety would not be compromised and security
would be best served, if a dead bolt lock were permitted to be installed
on the egress door toward Federal Street.
This approval rests with the Salem Building Inspector and has been
granted as of this date, per agreement reached after said inspection.
