Governor Conwdssioner June 3, 1996 Mr. Harrington RE:SALEM - Public Water Hawthorne Hotel Supply Cross Connection 18 Washington-Street Proposed Double Check Valve Salem, Massachusetts 01970 Assembly Installation at Hawthorne Hotel PWS I.D. #3258000 Transmittal #110732 Dear Mr. Harrington: The Department of Environmental Protection (DEP), has received your plan requesting approval on the proposed water supply cross connection installation mentioned above. The plans are titled: Hawthorne Hotel - 18 Washington Street Backflow Preventer The information submitted shows that it is proposed to install an Ames 2000SS double check valve assembly backflow preventer on a four (4) inch cold water line to a fire protection system. This device is located in the basement. The plans are hereby approved with the following understanding: 1. The double check valve assembly shall be installed with adequate space to facilitate maintenance, inspection, and testing. 2. The check valves and shut-off valves must be installed in a horizontal alignment with the floor. The top of the check valves must be a minimum of 30 inches and a maximum of 54 inches from the floor, unless otherwise approved by the Department. 3. Tightly closing valves must be installed at each end of the device. 5. Check valves must be provided with suitable connections and appurtenances for testing. 6. The owner or owner's agent must maintain a spare parts kit and any special tools required for removal and re-assembly of devices. 7. The owner or owner's agent must provide necessary labor for the inspection and testing performed by the Department or the supplier of water. 8. The water lines affected may be shut down during normal business hours and after reasonable notice to permit the necessary testing and maintenance of the double check valve assembly. The owner is responsible for making suitable arrangements regarding the shut down of these water lines. 9. The check valves must be protected from freezing, flooding, and mechanical. damage. 10. A permit from the local fire department shall be procured prior to the installation of a backflow prevention device on a fire protection system, pursuant to M.G.L. c. 148, s. 27A. In addition, the Department requires the installation be completed within 90 days after the receipt of this letter. Following installation completion, you will need to notify the local water department to make arrangements .for the initial inspection and test. As owner of this cross connection, you are responsible for compliance with Section 22 of the Drinking Water Regulations. Failure to take any action deemed appropriate by the Department or if you otherwise fail to remain in compliance in the future with the applicable requirements, you could be subject to legal action, including, but not limited to, criminal prosecution, court-imposed civil penalties, or civil administrative penalties. A civil admi ustrative penalty may be assessed by the Department for each day you are in noncompliance with the requirements referred to above. The Department strongly recommends obtaining a copy of 310 CMR 22.22 for information on legal responsibilities associated with installation of this device.
