Our goals were to: • Reduce the potential for property damage and loss of life as a result of coastal storm flooding; • Make the Building Code more consistent with the requirements of the National Flood Insurance Program; • Promote more consistent decisions between building officials and conservation officials and • Better protect wetland resource areas from the effects of buildings within them. The result of this effort is these regulatory revisions to the one and two family building code that is effective as of January 2, 2008 for which we are planning training workshops. Enclosed for your use are the following: • A summary of the changes to the 7`s Edition of the Massachusetts State Building Code; • A CD containing"Coastal Wetland Resource Maps" of your town and bordering communities; • Instructions.on how to use the CD; • A copy of an outreach brochure,flow chart and matrix that you can use to explain the regulatory changes and a PDF version for reprinting is also provided on the CD. • Who to contact if you have any further questions. I then called Disko aRa our engineer's visit in order to give them our initial observations,and we made arrangements to meet again this morning. We understmtd that the scope of work for Dialio's involvement as well as for Structures North's role is to mala the retaining wall safe Som firrdw collapse and to have the diagonal bracing that Diaho installed yeatadayremoved fiom the street. Observatlont The wall that collapsed is an improvised retaining wall consisting of a 61-4"high masonry fence on top of a 12"thick up to 10'-0"high unreinforced concrete wall. The wall is buttressed on the Vista Ave aid a by tapered concrete wing walla that arm 12"thick by 2'-2"at the base. There are two of these walls at 16'-0"on center, and then the last run of the tallest portion of the wall is unbraced by liattrmmes for the southerly 20'-0"of wall. The buttresses are also presumably constructed of unreinforeed concrete. The base of the retaining wall site on top of a projection that varies from 2'-10"high,to 3'-6"high. This projection is an earth filled planting bed for part of its length, and is a solid concrete base for the taller end. We took meanmmneats ofplumb along the wall and determined that at the free end of the wall it is out of plumb 3"iu 4'-0".Extrapolating that wall tilt over the retained soil height of the wall is out 71/2"in 10'-0". Extrapolation that wall tilt over the retained soil height plus the masonry fence on top of the wall is out 121/2"in 16'-41/2". The portion c f the wall that collapsed is at the high end that retained approximately 10'-0"of soil and that retr¢aed 90 degrees from Vista Ave. The South high most out of plumb and of the wall is the portion most in danger of further collapse. At this end of the wall Diaho has installed three timber Stumm that braced back to holes in the pavement and that are fastened to a vertical wood cleat on the f toe of the wall. Please note that Structures North has not evaluated the timber frame bracing;provided by Diahto. We have made some recommendation on site for improving this bracing. T 9 e bracing provides some margin of temporary protection, but the stabilization technique described by Structures North follows in the section under "Recommendations," below. Upon the coli apse of the south retaining wall that returns 90 degrees from vista Avenue,the soil has sloughed toward the south and has surcharged a smaller concrete block retaining wall that runs along the Vista Avenue sidewalk. This smaller wall has tilted and is itself in danger of collapse. The cause of the mining wall collapse was insufficient capacity of the marginally buttresses unrembrced contrera wall precipitated by the lateral earth pressure of the saturated cohesive soils behind the wall that are insufficiently drained. 2 DEC-10-2004 01:0411Y Fft-Structurar North 16787456067 T-246 P.004/004 F-31 4 Vista Avanue 10 December 2604 Slam,Massachusette Structures North Consulting Enoom Recommendations The following recommendations are based on stabilization and not on permanent repairs to the retaining wall. As discussed with Dialio,the concern of the City is in making the wall safe and secum and it will be the property owner's responsibilhy to have desianod and implemented the permanent repairs to the retaining wall. The stabilization recommendations will result in a stable temporary condition that does not require bracing onto the City stred or sidewallc The top 4'-0"of soil,x 4'-0"wide should be removed from the backside of the wall in order to relieve the Ward earth pressure. This requires careful placement of the excavator at least 10'-0" away from the wall(close to the pool wall). It also requires that the excavator be moving away from the stockpiled soil. The 4'-0"excavation should leave a stable slope,and the excavated area should be pK tected from trespass except by authorized personnel. Note that once the excavation is made,it wi ll not be possible to re-acoess the high grade area with a backhoe with without first performing the permanent retaining wall repairs. The lowered grado behind the wall should be sloped 1/8"in 1'-0"toward the south with a slight swale and then should be tarped. Tarps should be placed from south to north should overlapped 8"so that weer runoff will continue downhill. The South comer of the wall should get a temporary brace consisting of a steel frame anchored to the wall and oupported on the existing concrete footing Y-6" above the sidewalk. This fiame essentially tapes the place of the buttresses over the end 20'-0"of laterally uebraced wall. Structures Ncrcdt will issue a sketch for this frame. The top masonry fence should be removed from the south 20'-0" erns of leaning retaining wall. The low mac>nry retaining wall should be removed and the grade behind it made stable with not more than a 1:1 elope toward the south. The surface of the sloping grade should be protected fium erosion using geofebric. We have disc ussed these recommendation with Dialio construction on site,and will follow up with Actchm and confirmation that the wall has been stabilized by the removal of 4'-0"of soil fivm behind the wall. I asked Assistant City Engineer, Joe Nerden to accompany me on the initial visit. Upon arrival, we observed the retaining wall collapsed onto the public way and remnants of the wall dangling above the roadway. At that time I requested Salem Police to respond with sawhorses to guard the area adjacent to the roadway. At that time,Joe Nerden and I spoke with the homeowner,who was unaware the collapse had taken place, and informed him verbally that he had an "unsafe structure"and would need to begin work within 24 hours to make the structure safe. The following morning, December 9, 2004, I was not available, but I instructed my Assistant Building Inspector, Joe Barbeau,to serve a letter upon the owner, Walter Richards,reaffirming the unsafe structure in writing. The letter was served by Constable, Dan Stanwood, in hand to Mr. Richards. Mr. Richards indicated that he was unable to hire a contractor. Because of threat to public safety, my assistant,Joe Barbeau retained the services of Dalelio Construction Co, Inc. of Swampscott to begin to "make safe"the retaining wall. Dalelio took immediate steps to safeguard the area including hiring a Structural Engineer, Edward Moll, P.E. of Structures North to design an immediate short term repair and a design to make the wall safe for the immediate future. The worked commenced on December 9, 2004 and is ongoing. The final braces are being fabricated and will be installed early next week. At that time the temporary braces that are installed on the public way will be removed. `r The work to date totals approximately $15,000.00. The final cost is estimated to be $25,000.00I will supply your office with all the invoices and bills. Please advise, or take steps, to protect the City's funds that will be spent to make the wall "safe". Thank you for your help and we will supply any additional documentation you may require. Observotimu The wall that collapsed is an improvised retaining wall consisting of a 6'4'high masonry femce on top of a 1::"thick up to 10'-0"high unteinforood concrete wall. The wall is buttressed on the Vista Ave aid a by tapered concrete wing walla that are 12"thick by T-2"at the base. There are two of these walle at 16'-0"on center,and then the last run of the tallest portion of the wall is tmbreced by lnrareaees for the southerly 20'-0"of wall. The buttresses are also presumably constructed of unreinforrad comcaete. The base of the retaining wall site on top of a projection that varies fitan 2'-10"high,to 31-6"high. This projection Is an earth filled planting bed for part of its length, and is a solid concrete base for the taller end. We took measurements of plumb along the wall and determined that at the free end of the wall it is out of plumb 3" in 4'-0".Extrapolating that wall tilt over the retained soil height of the wall is out 71/2"in 10'-0". Extrapolation that wall tilt over the retained soil height plus the masonry fence on top of the wall is out 121/2"in 16'-41/2". The portion c f the wall that collapsed is at the high end that retained approximately 10'-0"of soil and that returned 90 degrees from Vista Ave. The South high most out of plumb end of the wall is the portion most in danger of further collapse. At this end of the wall Diaho has installed three timber ffxmert that braced back to holes in the pavement and that are fastened to a vertical wood cleat on the fine of the wall. Please note that Structures North has not evaluated the timber fi+ame bracing;provided by Diolio. We have made some recommendation on site for improving this bracing. 7h bracing provides some margin of temporary protection, but the stabilization technique des enbed by Structures North follows in the section wider "Recommendations," below. Upon the ooll apse of the south retaining wall that returns 90 degrees from vista Avenue,the soil has sloughed toward the south and has surcharged a smaller concrete block retaining wall that runs along th,e Vista Avenue sidewalk. This smaller wall has tilted and is itself in danger of collapse. As discussed with Dialio,the concern of the City is in making the wall safe and secure,and it will be the property owner's responsibility to have designed and implemented the permanent repairs to the retaining wall. The stabilization tecommendstions will result in a stable temporary condition that does not require bracing onto the City attract or sidewalk. The top 4'-0"of soil,x 4'-0"wide should be removed from the backside of the wall in order to i relieve the lm:eral earth pressure. Tbis requires carefrtl placement of the excavator at least 10'-0" away from the wall(close to the pool wall). It also requires that the excavator be moving away from the stockpiled soil. The 4'-0"excavation should leave a stable slope,and the excavated area should be protected from trespass except by authorized personnel. Note that once the excavation is made,it wi 11 not be possible to re-access the high grade area with a backhoe with without firet perfnning the permanent retaining wall repairs. The lowered grade behind the wall should be sloped 1/8"in 1'-0" toward the south with a slight Swale and then should be tarped. Tarps should be placed from south to north should overlapped a" so that wager runoff will continue downhill. The South comer of the wall should get a temporary brace consisting of a steel frame anchored to the wall and supported on the existing concrete footing Y-6"above the sidewalk. This frame essentially talces the place of the buttresses over the end 20'-0"of laterally unbraced wall. Structures Ncalh will issue a sketch for this frame. i The top mascmy fence should be removed from the south 20'-0" end of leaning retaining wall. The low mam)nry retaining wall should be removed and the grade behind it made stable with not more than a 1:1 slope toward the south. The surface of the sloping grade should be protected from erosion using geofabrlc. We have discussed these recommendation with Dialio construction on site,and will follow up with sketcher and confirmation that the wall has been stabilized by the removal of 4'-0"of soil from behind rhe wail.
