Dear Property Owner
At a recent inspection of 12 Union Street, I observed that flashing has not been installed
according to requirements 780 CMR Section 5703.2 at the heads of windows where
walls were re-sided and that new window trim on wall with with existing siding have also not
been properly flashed and this appears to be causing leaking into the wall cavity.
Please consider this notice of violation as required by 780 CMR 118.2.This condition
must be abated within 10 days of receipt of this notice under penalties described in 780
CMR 18.
You have the right to appeal this order to the State Board of Building Regulations at One
Ashburton Place, Boston, Ma.

Jeffrey Holloran as manager of Holloran Development, LLC, duly swears:
1. Holloran Development, LLC, is purchasing the building at 12 Union Street
(referenced as "the property,") in Salem, Massachusetts.
2. The property will be vacant at the time of closing.
3. The property will not be occupied prior to renovations.
4. The property is currently being utilized as a three-unit structure; however we plan on
obtaining permits to remodel back into a two-unit structure.
5. The property will not be occupied until all renovations are complete and all
appropriate approvals have been obtained by the City of Salem.
6. The foregoing affidavit consisting of one type written page is true and correct.
