As per our phone conversation this morning, I visited the site at 1 I Symonds Street that you have
your Construction Supervisors License (CS-028460) attached to, and it appears construction
work is beyond that you described has been completed.
This Department has no record of a framing inspection (Section 110.3.4 of 780 CMR, of the State Building
Code) nor is there a building pen-nit card displayed on the premise (Section 105.7 of 780 CMR, of the State
Building Code). No further work may be done until such time as the order is lifted. Any person who shall
continue any work in or about the building or structure after having been served with a Stop Work Order,
except such work as that person is directed to perform. To remove a violation or unsafe condition, shall be
liable to a fine of not more than $100 or by imprisonment for not more than one year, or both for each
violation; with each day constituting a separate violation.
You are hereby ordered to contact this office immediately to notify this office of the current state
of this project upon receipt of this notice. Failure to do so may result in further actions being
brought against you, up to and including the filing of criminal complaints at District Court. You
have the right to appeal this order to the State Board of Building Regulations at One Ashburton

Dear Sir:
Due to complaints of ungegistered motor vehicles in the rear yard of your proper-
ty at 11 Symonds Street in Salem, a site inspection was conducted on December 10,
1987 and the following were noted: t
Unregistered motor vehicles i.e, truck with white cab, blue/black van
blue convertible, yellow Pontiac and gold Sedan.
Your property which is on the hill abutts properties on Bryant Street. These
vehicles are at the edge of a ledge that separates your property from those
mentioned.
If any of these vehicles move, they will fall down into the back yards of houses
on Bryant Street causing serious property damage as well as possible serious or
fatal personal injury to anyone in the area.
You are hereby ORDERED to take immediate corrective action to remove these ve-
hicles from this residential area which are in violation of State Sanitary Code,
Chapter II, 105 CMR 410.602.
Failure on your part to comply within the specified time will result in a complaii
being sought against you in Salem Districe Court.
You are advised of your right to request a hearing before the Board of Health.
A request for said hearing must be received in writing in the office of the
Board of Health within seven (7) days of receipt of this ORDER.

three family dwelling. My husband and I lived on the first floor,
my brother resided on the second floor, and we rented the third
floor to a tenant , Shirley Knight. The property also included
a separate concrete, three car garage . We owned the house for
seven years , at which time we sold it to a Mr. Harold Walker,
still as an existing three family house .

I live at 15 Symonds Street in Salem,Massachusetts. It is a two family which has been in
my family since the early 1940's when my grandparents Cathryn and George Lord
bought the house. After my grandfather passed away in 1958 my family moved from
Chandler Street(around to comer)to 15 Symonds Street. To my knowledge 11 Symonds
Street is and always has been a three family home. I believe in the 1950's, i l Symonds
Street was owned by Mary and Donald O'Leary. Donald has since passed but Mary still
lives in Salem.
