Mr. Richardson,
The ensuing communication shall serve as our follow-up to our; site inspection on March 31,
2014, regarding unsafe egresses and no active sewage or water in the second floor unit and no
smoke detectors in the third floor unit at 67 School Street.
Upon completion of our inspection your property was found to be in violation of the
aforementioned complaints and in noncompliance with the Massachusetts State Building Code, therefore
this office finds reason to have the second and third ' floor Certificates of Occupancy removed.
Until such time that all requirements of this letter and any and all further requirements are
complete:
CERTIFICATE OF OCCUPANCY FOR SECOND FLOOR AND
THIRD FLOOR UNITS LOCATED AT 67 SCHOOL STREET IS
HEREBY REVOKED IMMEDIATLEY.
You are hereby ordered to begin to rectify these conditions immediately upon of receipt of this notice.
Failure to do so may result in Municipal tickets and further actions being brought against you, up
to and including the filing of criminal complaints at District Court.

REQUIRED INSPECTION NOTICE
67 School Street

Mr. Richardson,
Our office received a complaint regarding life safety and sanitary conditions at your property located at
67 School Street. The complaints were investigated on Friday, March 28, 2014 present were Mr. David
Greenbaum and Elizabeth Gagakis of the Salem Health Department and myself. Said property was found
to be in violation of numerous Massachusetts State Building Code requirements.
The most concerning off these violations that was noted during the inspections are the life safety and egress stairway
violations, including no snnoke detectors or Cot detectors in the second floor unit or common spaces;further I noted no
active water or no active sewer system on the second floor= For these reasons a Required Inspection must be conducted
by our inspection team to assure compliance with the code and city ordinances. It is requested that all egress stairways
and exit doors be cleared immediately of tiny and all storage, debris and other items.
Under the provisions of 780 CNIR, Section 104.6—Right of Entry, of the State Building Code, access to this property
must be granted for the purposes of this inspection. Please call this office upon receipt of this letter to schedule this
required inspection. I this property has rental units these tenants must be notilled in advance ofthis inspection, sa

This Required Inspection shall be conducted by this office on Monday, March 31, 2014 at 11:00 a.m. as noted above.;
failure to respond to this notification will be construed as non-compliance, and as such an Administrative Search
Warrant will be sought, so as to allow the lawful inspection of this property.
