PLEASE BE ADVISED,ON TUESDAY,MARCH 12,2002,I CONDUCTED AN ELECTRI-
CAL INSPECTION AT THE TANNING FACILITY LISTED ABOVE AND FOUND THE FOLLOWING
VIOLATIONS:
1.ROOM 1,4,2,7 AND 8: EMERGENCY SHUTOFF LABELS SHALL BE CLEARLY
MARKED AND HIGHLY VISIBLE.
2.BOOTHS 6 AND 10: EMERGENCY SHUTOFF LABELS FOR LAMPS HAVE TO BE
ATTACHED.
3.ROOM 5: IMPROPER WIRING METHOD WAS USED TO CON-
NECT TO THE TANNING BED.
5.ROOM 1: IMPROPER WIRING METHOD WAS USED TO CON-
NECT TO THE TANNING BED.
6.ROOM 10: THE CEILING BEHIND BOOTH 10,THERE ARE OPEN SPLICED WIRES.
KNOCKOUT SEALS ARE NEEDED TO FILL ALL UNUSED
OPENINGS IN ELECTRICAL BOXES.
7.EMERGENCY LIGHTS: ADJACENT TO THE RECEPTION DESK WAS ALSO WIRED
IMPROPERLY.
PLEASE BE ADVISED MASSACHUSETTS ELECTRICAL CODE ARTICLE 300 WIRING METHODS
AND ARTICLE 422-30 DISCONNECTING MEANS ARE BEING VIOLATED AT THIS TIME.
PLEASE HAVE A LICENSED ELECTRICIAN ACQUIRE A PERMIT FROM THE ELECTRICAL
DEPARTMENT TO PERFORM THE WORK MENTIONED ABOVE.FAILURE TO COMPLY WITH THIS
REQUEST WILL RESULT IN FURTHER ACTION.

Assessors Parcel #35-0606:
Dear Mr. Cahill :
Please be advised that the use of 2 Salem Green as an address for the above
parcel is not now, and never has been, recognized as a valid street address.
This parcel has no frontage on Salem Green; in fact, at its closest point,
it is over 130 feet away from Salem Green.
The only address officially recognized by the Cityj s 33 Federal Street. In
addition to Federal Street, the property has frontage on North Walkway, a
public way which was officially accepted at a special meeting of the City
Council on July 24, 1978. The building's entry fronts on this walkway. The
unauthorized assignation of the Salem Green address resulted in the formaliz-
ation with the City's Code of Ordinances of the traditional address assigning
procedure. This ordiance states that the Board of Assessors has sole
authority to assign street addresses in the city.
Salem Green is an unaccepted pedestrian walkway running from Church Street
between the Lyceum Restaurant and One Salem Green, behind the Lyceum and 81
Washington Street, to the walkway at the rear of City Hall . At no time has
the name Salem Green been used to designate the walkway on which this parcel
fronts.
