Notice is hereby given that the City of Salem,Massachusetts shall accept"Requests for
Proposals"(RFP's) for the replacement of one (1) exterior door unit with two (2) sidelights and
transom panel located at the rear handicapped ramp entry at the Salem Senior Center, 5
Broad Street, Salem, Massachusetts. This project is federally funded and shall conform to
the current Davis - Bacon prevailing rate schedule.
Proposals must be received at the Public Properties Department, Salem City Hall Annex, 120
Washington Street, 3`d Floor, Salem,MA 01970, on or before 11:00 AM on Friday,November
13,2015 at proposals will be reviewed and the winning bidder notified.

The RFP award is made by the Public Properties Assistant and is subject to Designer Selection

INTENT OF CONTRACT
The City of Salem is seeking to purchase the services to remove the existing door and frames fabricate,
deliver and install,one (1)exterior door unit with two (2) sicelights and transom panel as specified
in the Bid Documents for the Salem Senior Center located at 5 Eroad Street, Salem, Massachusetts.
The RFP shall describe a complete project with all manufacturer,' standard equipment. The City seeks to
achieve competitive pricing for similar services from various manufacturers/Contractors.
The specifications included with the RFP shall describe the minimum qualifications to fit our needs.
Products,which meet and/or exceed these qualifications,may be considered.
The City of Salem is an"affirmative action-equal opportunity employer"and encocrages all MBE's,

This project is being funded under the Office of Policy and Management Municipal Energy Efficiency
and Conservation Block Grant. Pursuant to the American Recovey and Reinvestment Act(ARRA of
2009,PUB. L 111-5, AS AMENDED,there are special conditions that shall be adhered to and satisfied
by the successful Bidder.They are,but not limited to:

alteration, maintenance,or repair of a public building or public work unless all of the iron, steel,and
manufactured goods used in the project are produced in the Unitec States. All submittals for products

This is a prevailing rate job. Successful Contractor shall supply Certified Payrolls on a weekly basis to the
Owner.
Any subcontractor or sub grantee under this agreement shall retain and maintain accurate records and
documents relating to performance of services under this agreement for a minimum of three(3)years
from the expiration of the subcontract or subgrant and shall make them available for inspection and audit
by OPM or its representative.
The fabricated doors shall be of the same design of the existing wood doors on the front of the Senior
Center. All doors and frames shall be"factory painted" in a control-ed environment to match the existing
doors. All hardware shall be of the same manufacturer and quality as the existing hardware. All locks
shall be keyed the same to work with the current buildings key system,to be purchased and coordinated
with the Owners representative..

GENERAL CONDITIONS
1. `1'he City of Salem reserves the right to reject any cr all RFP's, if deemed in the best
interest of the City.
2. Minor variations to the specifications not affecting the performance may not disqualify' a
RFP from consideration. The Designer Selection Committee or its designee will make
that determination.
3. All Federal and State laws and regulations must he complied with.
4. Bid documents shall include a delivery timeframe. Penalties for not complying with said
timeframe shall be at a rate of$50.00 per day.
5. Upon notification of Award,the successful bidder shall provide a Certificate of Insurance
to the City of Salem, showing the City of Salem, the Design Review Committee and the
State of Massachusetts as additionally insured.

I. DOOR UNIT SIZES: Approximately 6'-0 '/s"x 9-3 1/4", one (1) exterior door unit with two
(2)sidelights and transom panel.

Side Lights two (2) approximately 1'-2 %a"x 7'-0"- Size and style to be verified in field.

2. DOOR TYPE: 18 ga., galvanized, insulated hollow six panel metal doors with pre painted in
a factory environment to match existing door color.
3. SIDELIGHT TYPE: 18 ga., galvanized, insulated hollow !hree panel metal sidelights with
pre painted in a factory environment to match existing door_-olor.
4. TRANSOM TYPE: 18 ga., galvanized, insulated hollow four panel metal transom with pre-
painted in a factory environment to match existing door color.
4. DOOR FRAME: l6ga. Galvanized welded hollow metal frames (to be veriged infield)
5. HARDWARE: Industrial grade hardware, including hinges,push bar panic hardware,
closers, thresholds, removable aluminum mullion (where applicable) and lever handles to match
existing.
6. QUANTITY: one (1) exterior door unit with two (2) sidelights and transom panel.
7. MISCELLANEOUS: Doors to have weather stripping, brush door sweeps, and all necessary
wood trim painted to match existing trim, removed during the project.
All doors and frames are to be painted in a"factory setting"with a controlled environment.
All materials and construction methods must conform to local and State building codes. All
permits shall be the responsibility of the successful Contractor,applicable fees will be waived.
The City of Salem reserves the right to limit the quantity of doors purchased to stay within
budgetary limits.

PROCEDURES FOR FILING APPLICATIONS
A. Be prepared to apply for approvals well in advance of commencing any exterior work. Before makings changes to the exterior of a property
in an historic district,the owner should call or visit the Commission representative at the Department of Planning&Community Development to
discuss proposed alterations and to determine the category of the application(Appropriateness,Non-Applicability or Hardship).
B. The Commission normally meets on the first and third Wednesdays of each month and notices are posted at City Hall. The meetings are held at
120 Washington Street,3rd floor,and begin at 7:00 p.m. All meetings are open to the public and any person is entitled to appear and be heard
on any matter before the Commission before it reaches a decision.
C. An application for a Certificate of Appropriateness or Hardship must be received by 3:00 p.m.on the Monday 16 days before the meeting in
order to make the agenda. An application for a Certificate of Non-Applicability may normally be added to the agenda up to the day of the
meeting. In some cases,a Certificate of Non-Applicability maybe issued by the Clerk,without review by the full Commission,after notifying
Commission members and allowing 24 hours for any objection. There is no fee for any applications.
D. All applications for Certificates of Appropriateness or Hardship require a public hearing. Notice of the hearing must be posted with the City
Clerk 14 days before the hearing and abutters must be notified in writing. Commission staff will handle these procedures. A public hearing is
not required for a Certificate of Non-Applicability.
E. Applications must be submitted by the owner of the property. A contractor for the owner,at the owner's request,may submit an application on
the owner's behalf. In case of a tenant,a waiver of the owner's appearance maybe granted at the discretion of the Commission if it is requested
by the owner.
F. All applications must include photos of existing conditions,taken from all public ways. No certificates can be issued until receipt of photos.
G. An application will not be considered complete unless all work items are thoroughly described on scaled drawings and include specifications
regarding dimensions, materials, and any other information needed for the Commission to visualize the changes in order to make a
determination. Applications for paint colors should include a paint chip or chart. The following items should be included in your drawings as
applicable(exhibits should be 8 %2 x 11 or be able to be folded to 8%2 x 11):
1. Site plan showing location of improvements;
2. Elevation drawings of the specific improvements;
3. Detailstprofiles(i.e.moldings,fence caps,cornices,vents,etc.);
4. Materials(i.e.wood,brick,etc.);
5. Dimensions(i.e. size of trim);and
6. Transformers,heat pump and condenser locations,electrical entries and meters,lamp posts,stove pipes.
H. At the hearing,the Commission will discuss the application with the applicant or his representative,hear the abutters and take a vote. Owners
having professional consultants such as architects or contractors are urged to have them be present at the hearing. If the application is approved,
a Certificate will be mailed to the mailing address provided on the application and copies will be sent to the City Clerk and Building Inspector.
Please note that the application can be continued until the next meeting if the Commission deems necessary(i.e.for reasons of incomplete
drawings,to perform a site visit,etc.). In any case,the Commission must make a determination within 60 days from the date the application is
received,unless the applicant waives that requirement in writing.
I. A property owner or a contractor cannot receive a building permit unless a Certificate has been issued. Please be sure to obtain appropriate
permits from the Inspector of Buildings(or other necessary permits and approvals)prior to commencing work.

K. The City of Salem reserves the right to inspect the project to determine compliance with the conditions set forth in the Certificate issued.
Violations
A person commencing or completing work to the exterior of a building in an historic district without the necessary approval of the Commission is subject
to fines of up to$500 per day from the date of violation. The Commission is not responsible for an owners neglect to inquire about necessary City permits
and approvals. All records are public and we will confidentially assist you if you are concerned that someone in your neighborhood is in violation.

Public Library or on-line at hup:Hwww.salem.com/Pages/SalemMA Historic/index. The guidelines provide examples of what is historically appropriate
(or inappropriate) for Salem's neighborhoods including trim, siding, barrier free access, doors, fences, gutters/downspouts, masonry, mechanical
equipment,paint colors,parking solutions,porches/steps,roofing,satellite dishes&solar energy systems,secondary egress,skylights,utilities,windows,
etc. These guidelines help you understand what changes are likely or unlikely to be approved. The Commission supports your efforts to improve your
