Tremblay Director of Public Property Inspector of Building Zoning Enforcement Officer March 3, 1997 Stephen Malik 28 Varney Street Salem, Mass. 01970 RE: 12-16 Proctor Street Dear Mr. Malik: On Friday February 28, 1997, I was asked to conduct an inspection of the above mentioned property by Fire Inspector Norman LaPointe, and the following violations were found: 1 . Structural engineers report that the floors are capable of carrying existing loads. 2. Stairwell at front of building requires rails, self closing doors, emergency lights and protection from glass windows. 3. Fire extinguishers must be refilled and updated. 4. All debris must be removed from building. 5. Building must be made accessible. 6. Work areas must be cleaned up and made workable. 7 . Gas heaters must have proper tag displayed. 8. All electrical work must be done by a licensed electrician. 9. Emergency lighting must be installed in all working areas. 10. All faulty wiring must be removed. 11 . All plastic installed on sidewalls and ceiling must be removed. 12. Contractors working on building must pull proper permits. 13. Thirty-six inches (36" ) is the required aisles at storage shelving. 14. No materials shall be stored any closer than twenty-four (2411 ) to a sprinkler head. 15. Business Certificates must be obtained. 16. Certificates of Inspections must be obtained. 17. The businesses that are occupying this structure must obtain special permits from the City of Salem Board of Appeals. The structure is a legal "Grandfathered" use as a Tannery and any other use of the structure must have approval by the City of Salem Board of Appeals These existing violations must be corrected as soon as possible to insure safety of the workers and of the structure. This office will allow reasonable time for the necessary repairs, but if not satisfied with ongoing process will file legal action against you and property business owners. Business Certificates must be obtained. 16. Certificates of Inspections must be obtained. 17. The businesses that are occupying this structure must obtain special permits from the City of Salem Board of Appeals . The structure is a legal "Grandfathered" use as a Tannery and any other use of the structure must have approval by the City of Salem Board of Appeals These existing violations must be corrected as soon as possible to insure safety of the workers and of the structure. This office will allow reasonable time for the necessary repairs, but if not satisfied with ongoing process will file legal action against you and property business owners.
