YOUR ATTENTION IS HEREBY DIRECTED to the provisions of
Chapter 2, Article XV et.seq. of the 14unicipal Code of the
City of Salem, Massachusetts, on file in the office of the
City Clerk on the first floor of City Hall .
Pursuant to the provisions of said Chapter 2, Article XV,
you are hereby notified that a certain unsightly condition
exists on premises specifically described at the facade
surroundine 26 Prince St. .Salem. Massachusetts 01970.
which iniures neighboring property and the public health,
safety, and welfare. You are therefore notified at once.
and in anv event within thirty (30) days from the date of
this notice, to keep said property free therefrom.
In the event you fail to complete such work within the
time hereinabove mentioned, the undersigned shall cause
the same to be removed and you will be responsible for the

Plans submitted for review on Sept. 15, 1981, for substantial
renovations to the structure at 26 Prince St,/33 Park St. are
not approved, as submitted.
The following items shall be noted and new plans submitted with
corrected data.
1. Plan E-lA
a. Oil burner switches shall be located just outside the boiler
room. You may keep additional switches in the apartments if
desired.
b. Smoke detector shall be located at base of each stairway in
basement. Location shown on plan is not at base of stairs.
c. Each open basement area shall require one smoke detector.
This may replace one heat detector now shown in open basement.
Adjust diagram accordingly.
2, Plan E-2A
a. Smoke detectors shall be placed appropriately at the base of
each stairway, as it leads up to second floor, but not within
the stairway.
b. A silence alarm switch shall be provided on the fire alarm
panel, with a key provided the fire department. This shall
be so noted on the plan.
3. Plan E-3A
a, Smoke detector to be placed at base of each stairway, and
at the top of each stairway. Not as shown in hall area.
b.. Smoke detectors located within each apartment unit shall be
single station type, and shall be so identified on the plan
and a symbol for same shall be added to the legend, on Plan EIA.
c. Relocate single station smoke detector, in apartment hall
approximately four feet into hall, so as to provide adequate
protection for the third bedroom.
4. Revised plans should be submitted for review.
5. A form #81 shall be filed by the contractor or architect for
obtaining stamped approval of the plans, and before application

is made for a building permit. Three sets of plans are required
for stamped approval.
6. A Form #81F must be filed by the electrical contractor, prior
to initiating work. A Form #82 (Certificate of Completion) ,
vsill be required upon completion and at final inspection.
