In accordance with provisions of Section II-B of the Salem Subdivision Regulations, the applicant must
file, by delivery or registered mail, a notice with the City Clerk stating the date of submission for such
determination. The notice shall be attached to this application form. Seven (7) hard copies of the notice
and application shall be brought by the applicant to the City Clerk to be "date stamped". The City Clerk
will retain the original; the remaining six (6) hard copies of the notice and application, as well as six (6)
hard copies of the Plan, and one (1) electronic copy of the notice, application, and plan, shall be filed in
.pdf format with the Planning Board by the applicant. All hard-copy notices and applications shall be
typewritten or neatly printed in ink.

Dear Amanda:
I am writing to follow up our phone conversation regarding the Approval Not Required
(Form A) Plan for the above referenced property.
As I mentioned, because the property is registered land, the plan signed previously by the
Planning Board had to be revised by the land surveyor to meet the technical filing requirements
of the Land Court. However, there have been no changes to the lot lines or dimensions of the two
lots created by the plan. The new plan simply includes much more detailed survey information.
I have attached copies of both the earlier plan and the new plan. I would appreciate it
very much if the Planning Board could endorse the new plan at the meeting on July 21, 2016.
Thank you for your assistance and please let me know if you have any questions or
concerns.
