I hereby certify that, pursuant to Massachusetts General Law,
the Salem Planning Board has taken that following action with
respect to the above referenced subdivision.
By a vote of five in favor, none opposed, the Planning Board
approved the proposed subdivision plan as presented with the
attached conditions.
Notice of the Planning Board's action and the reasons
supporting it are more fully described in the attached copy of a
letter, the original of which was sent to the applicant by
registered mail, postage prepaid, at his address stated on the
petition.

Dear Ms. Lutts & Mr. Wilensky:
The Salem Planning Board voted on November 7 , 1991, to approve
the proposed subdivision plan entitled "Subdivision Plan, Salt Wall
Lane" dated September 23, 1991 , revised November 7 , 1991 , subject
to the following conditions :
1. Conformance with Plan
Work shall conform to plan entitled "Subdivision Plan,
Salt Wall Lane" dated September 23 , 1991 , revised
November 7 , 1991.
2 . Utilities
Utility installation shall be reviewed and approved by
the City Engineer .
3 . Sidewalks/Planting Strip
A five foot sidewalk and a three foot planting strip
shall be constructed along frontage of lot.
4 . Granite Curbing
6 inch vertical granite curbing shall be installed along
frontage of lot.
5 . Landscaping
A minimum 3 1/2" caliper tree shall be planted in the
planting strip. The tree shall be compatible with
existing street trees on Salt Wall Lane.
6 . Board of Health
•
Board of Health requirements set forth in a letter dated
October 10, 1991, shall be strictly adhered to.
7 . Clerk of the Works
A Clerk of the Works shall be provided at the expense of
the developer as is deemed necessary by the City Planner .
8. Construction Practices
All construction shall be carried in accordance with the
following conditions:
a. No work shall commence before 7 : 00 a.m. on weekdays
and 8:00 a.m. on Saturdays. No work shall continue
beyond 5:00 p.m. There shall be no work conducted on
Sundays or holidays. Inside work of a quite nature may
be permitted at other times.
b. All reasonable action shall be taken to minimize the
negative effects of construction on abutters. Advance
notice shall be provided to all abutters in writing at
least 72 hours prior to commencement of construction.
C. Drilling and blasting and hoe ramming shall be
limited to Monday-Friday between 8: 00 a.m. and 5: 00 p.m.
There shall be no drilling or blasting on Saturdays,
Sundays or holidays.
d.. All construction vehicles shall be cleaned prior to
leaving the site so that they do not leave dirt and/or'
debris on roadways as they leave the site.
e. All construction shall be carried out in accordance
with the Rules and Regulations of the Planning Board.
9. As-Built Plans
As-Built plans, stamped by a Registered Professional
Engineer or land Surveyor, shall be submitted to the
Planning Department and Engineering Department prior to
issuance of Certificates of Occupancy.
10 . Violations
Violations of any conditions shall result in revocation
of this permit by the Planning Board.

This endorsement shall not take effect until a copy of the
• decision bearing certification of the City Clerk that twenty ( 20 )
days have elapsed and no appeal has been filed, or that if such
appeal has been filed, and it has been dismissed or denied, is
recorded in the Essex county Registry of Deeds and is indexed in
the grantor index under the name of the owner of record or is
recording or registered shall be paid by owner of applicant .
I hereby certify that a copy of this decision and plans are on
file with the City Clerk and a copy is on file with the Planning
Board.
