This letter shall serve as notification that on Monday,August 13,2015 your property at 13 Pleasant Street
was inspected along with the Salem Health Department and was found to be in violation of local zoning
ordinance and Massachusetts State Building Code. Your property is legally a two (2)—unit building upon
our inspection an illegal third(3rd)unit appears to exist on your property.
The rear deck is in disrepair,the smoke detectors in all areas of the building are missing or non-operable;
and blocked exits were noted.
You are hereby ordered to contact this office immediately upon receipt of this letter. Our office
has no records of the illegal third unit being constructed. A Building Permit to remove said
illegal unit with a Construction Supervisor Licensed (CSL)individual applying for the permit
shall be required.
You are hereby ordered to apply for a permit within 15 days of receipt of this notice. Failure to do so may
result in further actions being brought against you, up to and including the issuance of Municipal tickets or -
filing of criminal complaints at District Court. You have the right to appeal this order to the State Board of
Building Regulations at One Ashburton Place, Boston, Ma.

This letter shall serve as notification that on Tuesday,April 4,2017 your property at 13 Pleasant Street
was inspected by this office and was found to be in violation of local zoning ordinance and Massachusetts
State Building Code. As stated in our prior letter dated August 25,2015 your property is legally a two(2)
—unit building upon our recent inspection as with our inspection of 2015 an illegal third(3rd)unit exists
at your property.
As noted during this latest inspection, the illegal third floor unit at your property was occupied, you must
cease and desist this activity immediately. A review of the second-floor unit revealed that unpermitted
electrical work had been performed. A review of City records show no Electrical permit has been issued
for any work on that floor level.
It is unclear whether the rear decks that were in violation in 2015 have been repaired at the release of this
letter. Our Department has issued a permit(B-15-957)but it appears no inspections have been requested
or completed at the property.
You are hereby ordered to contact this office immediately upon receipt of this letter. Our office has no
records of the illegal third unit being constructed. A Building Permit to remove said illegal unit with a
Construction Supervisor Licensed(CSL)individual applying for the permit shall be required.
Once again are hereby ordered to apply for a permit immediately upon of receipt of this notice. You have exhausted the
time frame in which to appeal the initial complaints through the proper agencies.The Building Code violations through

Pleasant St. They were on the scene of a medical aid with NSA.on floor two. They reported
seeing several dangerous conditions within the dwelling. I did a quick walk thru with these
officers. I witnessed the following;
1. Several detectors were found hanging from the ceiling and not working.
2. No detectors were found in the occupied third floor bedrooms
3. The third floor area was being occupied by a young lady with a small baby.She stated
that she- was not related to the owners family but only
a friend living there. This third floor (attic) area has only one way out ! !
4. £xtesnsion cords were all over the place.

6. There was a strong smell of animal urine and feces.
7. I wa.s unable to enter the collar due to pit bulls located there. The smell proved it.

Deputy Chief Brophy, in his report, cited several health and safety concerns including
missing smoke detectors,blocked egresses and several electrical violations.
Under the authority of the State Building Code, 780 CMR, Section 103, an inspection of
your property has been scheduled for Wednesday, October 2 at 1:30 p.m. You are
directed to have all areas open and available for inspection.

Dear Owner:
Yesterday,October 2, a required inspection of you property was conducted.
Your son, Mark Hobart was present for the inspection. Representatives from Health,Fire
and Animal Control Officer were present. The Health Inspector and Animal Control
Officer will issue their own reports.
My findings were the following:
1. Smoke Detectors;Missing and disconnected in several areas throughout the house.
This problem poses an immediate threat to life and must be corrected within 24 hours.
2. Common hallways; Obstructed with plants,clothing and other items. Mass State
Building Code 780 CMR,does not allow the common hallways to be used for any
Purpose other than an egress. All items must be removed in 24 hours.
3. Large amounts of combustible clothing that completely fill the downstairs apartment
and some areas on the second floor. Work to begin immediately to remove these items
and to be complete within 30 days.
4. Middle room on third floor not legal to continue use as a bedroom. Room does
not have required egress window. Needs to be vacated immediately.
5, No more than three(3) unrelated people can occupy the second and third floor
suite. You are directed to provide the names and relations of all occupants.
6. Unregistered and uninspected vehicle to be removed within 15 days as per our
discussion.

3-Story, 2-Family residential structure
Per 2009 IEBC Section 104.2.1.1 I have done a walk-through and noted the 3-Unit situation that
currently exists. Some minor adjustments need to be made to the house such that it is
appropriately used as a 2-Family residential building. My evaluation follows below:
Removal of 3rd Floor Vitchen:
Removal of the 3rd Floor cooking range is required. Refrigerator and sink can remain.
3rd Floor Ceiling Heights, Stairs & Windows:
At 3rd Floor existing ceiling height at 6'-3" and winder stairs are Existing/Non-Conforming
and are acceptable as-is. Bedroom window sizes accepted as Existing/Non-Conforming.
2nd Fir Rear Egress:
Doors shall be removed at the base of the 3rd Floor Rear Stairs and at the 2nd floor kitchen.
At the base of the 2nd Floor stairs an outswinging door will be installed that will be the rear
entry/egress door to Unit 2 (Unit 2 shall be comprised of the 2nd and 3rd Floors).
1st Floor Rear Vestibule:
The existing 1st Floor Rear Vestibule exterior door will be reversed to swing OUT so as not
to obstruct the swing of the rear entry door of Unit 2 which is at the bottom of the stairs.
Rear stair landing:
The rear entry/egress stair landing will be extended so that there is a clear 36" landing where
the exterior door swings out.
Porch Structure modification:
To accommodate the stair landing extension a central post for the 2nd floor porch will be
moved I foot away from it's current postion. The change in location does not change the code
compliant structure of the porch.
Fire Alarm System:
Consult with Licensed Electrician for verification of Code Compliant fire detection and
alarm system for entire 2 Family building.
Inspection of Completion by Architect:
The Architect will inspect work for completion when requested by home owner and report
findings to the Building Inspector
