Although it has not been stated as such, we assume that the proposed road off of Loring Avenue is to be public. No information on the proposed road has been presented. Profiles and utility plans should be provided. 2. Mr. Zieff should clarify just exactly what his plans for this development are going to be. We have already identified problems with inability of the existing water system to meet standard fire flow requirements for Phase II. Additionally these developments may have a significant impact on the sewerage system as well as traffic in the surrounding area. Prior to commencement of construction activity, a haybale barrier shall be placed downgradient of construction to prevent siltation of the adjacent wetland. Haybales shall be anchored with stakes and entrenched 4 to 6 inches in depth. Trench material shall be deposited on the upgradient side of the haybale barrier. Haybales shall be inspected daily,those showing signs of deterioration shall be replaced immediately. The haybale barrier shall remain in proper functioning condition until all disturbed areas have been stabilized. 6. Additional erosion and sedimentation control measures deemed necessary by the Commission shall be instituted by the applicant. Any siltation caused onto any wetland or waterway area shall be promptly reported to the Commission. Such areas shall be restored to the satisfaction of the Commission. 8. Granite or concrete curbing shall be installed around the parking lot perimeter to contain and direct the flow of water to catch basins. 10. *All catch basins shall have deep sumps and pipes shall be fitted with oil and gas traps. The entire drainage system, including street sweepings,catch basin clean outs, manholes, and maintenance of swales, sediment forebays, detention areas, shall be required to follow the attached approved Stormwater Management System Operation and Maintenance Plan as amended and be recorded as one with this Order. If, as, and when the City of Salem should accept Carol Way, Weatherly Drive, or America Way as a public street, the maintenance of the stormwater management system, as incorporated herein, excluding the storm drains, catch basins, and manholes located within the street layout, shall remain with the developer, its successors or assigns. 11. *A letter confirming the implementation of the cleaning and maintenance schedule shall be filed with this Commission biannually on June 15 and December 15. 16. The No Cut/No Disturbance Zone as shown an the plans (pre-construction) shall be extended eighty five feet(85') southerly and be enlarged to include the entire wetland resource area. The No Cut/No Disturbance Zone as shown on the plans and as expanded above shall be further expanded after construction is completed (post-construction) from top of haybale line upgradient easterly to the 70' topography line. The final No Cut/No Disturbance Zone (wetland resource area, expanded pre-construction, and post- construction zones)shall be combined as one and mathmatized on a modified plan C-6 for grading, drainage and erosion. No work shall be performed until copies of the modified plan are submitted to and approved by the Salem Conservation Commission. As-Built drawings shall show the enlarged,pre and post-construction zones as one mathmatized No Cut/No Disturbance Zone. Deed restrictions shall be placed on the individual deeds to reference and identify the final No Cut/ No Disturbance Zone. Individual deeds shall also include these Special Conditions. Copies of proposed deeds are to be submitted to and approved by the Salem Conservation Commission prior to the issuance of any Certificate of Occupancy permits. 18. *Fertilizers used for landscaping and lawn care shall contain a low nitrogen content and be used in moderation.
