MASS. February 28, 1980 SPECIAL PERMIT WETLANDS AND FLOOD HAZARD DISTRICT MR. AND MRS. JOSEPH BOUDREAU 409 LAFAYETTE STREET SALEM, MA. 01970 On Thursday, February 21, 1980, the Planning Board of the City of Salem held a public hearing regarding the application of Joseph and Joan Boudreau, for a Special Permit under Section P, .,etlands and Flood Hazard Districts, of the Salem Zoning Ordinance, with respect to the construction of a pier and dock at 409 Lafayette Street, Salem, Massachusetts. At a regularly scheduled meeting of the Planning Board on February 71, 1980, the Board voted, by a vote of seven in favor, none opposed, to approve the application as complying with the requirements for the issuance of the permit with the following condition: 1. The work shall be done in accordance with the site plan prepared by R. L. Mailhoit Associates , entitled "Pier & Float 409 Lafayette Street -- Salem, Ma. " I hereby certify that a copy of this decision has been filed With the City Clerk, and a copy is on file with the Planning Board. This Special Permit shall not take effect until a copy of the decision bearing the certification of the City Clerk that twenty days have elapsed and no appeal has been filed or that if such appeal has been filed, and it has been dismissed or dented, is recorded in the Essex County Registry of Deeds and is indexed in the grantor index under the name of the owner of record or is recorded and noted on the owner's certificate of title.
