Amanti Highland Realty Company, LLC 392 Highland Avenue Salem, MA 01970 RE: Self-Storage Facility, 394 Highland Avenue, Site Plan Review On Thursday, September 7, 2000, the Planning Board of the City of Salem opened a Public Hearing regarding the application of Thomas and Dorothea Amanti under Site Plan Review for the construction of a two-story self-storage facility containing a maximum of 333 units located at 394 Highland Avenue. In addition, the plan shows a parking lot consisting of 23 vehicle spaces. The Public Hearing was continued to September 21, 2000, October 19, 2000, November 16, 2000, December 7, 2000 and closed on January 4, 2001. At a regularly scheduled meeting of the Planning Board held on January 4, 2001, the Board voted by a vote of seven in favor, one opposed, to approve the application as complying with the requirements of Site Plan Review, subject to the following conditions: 1. Conformance with the Plan Work shall conform with the plans entitled, "Proposed Self Storage Facility at 392 Highland Avenue Salem, MA", "Site Plan for Highland Realty Co., LLC", sheets C-1, prepared by Griffin Engineering, P.O. Box 7061, 100 Cummings Center, Suite 3321, Beverly, MA 01915, dated 11/6/00 incorporating revisions 1. 9/20/00 "Revisions to Landscaping"; 2. 11/6/00 " General Revisions — Reduce Bldg. Amendments Any amendments to the site plan shall be reviewed by the City Planner and if deemed necessary by the City Planner, shall be brought to the Planning Board. Any wr pf. conditions contained within shall require the approval of the Planning Board. �HIIYV 25 A 00 3. Landscaping a. All landscaping shall be done in accordance with the approved set of plans. b. Trees shall be a minimum of 3 % inch caliper and with a minimum height of 8 feet. c. Maintenance of landscape vegetation shall be the responsibility of the developer, his successors or assigns. d. A final set of landscaping plans shall be submitted and approved by the City Planner. Particular attention should be paid to the vegetation proposed adjacent to Olde Village Drive. If plan is revised, the City Planner shall notify the manager of the adjacent condominiums. e. After such landscaping is completed, the developer will add further landscaping if such is requested by the Planning Board or the City Planner. 4. Signage a. Proposed signage shall be reviewed and approved by the City Planner and the Sign Review Committee. 5. Lighting a. No light shall cast a glare onto adjacent parcels or adjacent rights of way. b. A final lighting plan shall be submitted to the City Planner for review and approval prior to the issuance of a building permit. c. After installation, lighting shall be reviewed by the City Planner, prior to the issuance of a Certificate of Occupancy. 6. Retaining Wall a. A final plan for the front retaining wall (along Highland Avenue) shall be submitted and approved by the City Planner. It is recommended that the wall contain a cap. 7. Fences a. The fence located at the south comer of the front line of the storage facility shall have a height of 6 feet and extend approximately 15 feet. b. A final elevation of the fence to be constructed along Olde Village Drive Shall be submitted and approved by the City Planner. 8. Work shall not be conducted between the hours of 5:00 PM and 8:00 AM the following day on weekdays or at any time on Sundays or Holidays. 2001 JAN 25 A 4 00 b. All reasonable action shall be taken to minimize the negative effects of construction on abutters. Advance notice shall be provided to all abutters in writing at least 72 hours prior to commencement of construction of the project. c. Drilling and blasting shall be limited to Monday-Friday between 8:00 AM until 5:00 PM. There shall be no drilling or blasting on Saturdays, Sundays, or holidays. Blasting shall be undertaken in accordance with all local and state regulations. d: All construction vehicles shall be cleaned prior to leaving the site so that they do not leave dirt and/or debris on surrounding roadways as they leave the site. e. All construction shall be performed in accordance with the Rules and Regulations of the Planning Board, and in accordance with any and all rules, regulations and ordinances of the City of Salem. f. All construction vehicles left overnight at the site, must be located completely on the site. 9. Clerk of the Works A Clerk of the Works shall be provided by the City, at the applicant, his successors or assigns' expense, as is deemed necessary by the City Planner. 10. Board of Health All Board of Health requirements as specified in the attached decision dated September 19, 2000, shall be strictly adhered to. 11. Fire Department All work shall comply with the requirements of the Salem Fire Department. 12. Conservation Commission All work shall comply with the Order of Conditions issued by the Salem Conservation Commission. 13. Building Inspector All work shall comply with the requirements of the Salem Building Inspector. 14. Utilities a. Utility installation shall be reviewed and approved by the City Engineer prior to the issuance of a Building Permit. b. The drainage plan and calculations shall be reviewed and approved by the City Engineer prior to the issuance of a Building Permit in order to insure that the recommendations by the Assistant City Engineer related to a memorandum dated 1/4/01 to Don Cefalo from Joe 394 Highland Avenue, Self-Storage Facility CITY OF SALEM. Exterior Elevations a. Final elevations, brick type, and other building materials shall be reviewed and approved in writing by the City Planner prior to the issuance of a Building Permit. 16. Hours of Operation a. The hours of operation for the self-storage facility shall be 7:OOAM until 9:00 PM. Any amendments to hours of operation shall require the approval of the Planning Board. 17. Storage Restrictions a. The management of the storage facility shall prohibit the use of any type of business to be operated within and/or out of the confines of the storage facility. The storage facility will prohibit any inventory storage within the confine of the facility that would require the occupant (lessee) to have regular, daily access to the storage unit. b. All restrictions as stated in the attached listed titled "Proposed Language for Prohibited Items and Use", shall be strictly adhered to. c. A statement shall be added to the standard informing renters not to use Olde Village Drive as a means of a turn around onto Highland Avenue. 18. Maintenance a. Refuse removal, ground maintenance and snow removal shall be the responsibility of the developer, his successors or assigns. b. Winter snow in excess of snow storage areas on the site shall be removed off site. c. Maintenance of all landscaping shall be the responsibility of the applicant, his successors or assigns. The applicant, his successors or assigns, shall guarantee all trees and shrubs for a two- (2) year period. 19. As-built Plans As-built plans, stamped by a Registered Professional Engineer, shall be submitted to the Planning Department and Engineering Department prior to the issuance of the final Certificate of Occupancy. 20. Violations Violations of any condition shall result in revocation of this permit by the Planning Board, unless the violation of such condition is waived by a majority vote of the Planning Board. 394 Highland Avenue, Self-Storage Facility January 4,2001 -5 I hereby certify that a copy of this decision and plans has been filed with the City Clerk and copies are on file with the Planning Board.
