Joseph Correnti Serafini, Serafini, and Darling 63 Federal Street Salem, MA 01970 RE: Ridgeside Apartments, 205 Highland Avenue Site Plan Review On Thursday, February 17, 2000, the Planning Board of the City of Salem opened a Public Hearing regarding the application of Autumn Development Company, Inc. under Site Plan Review for the property located at 205 Highland Avenue. The proposed project includes the demolition of the former Vincent's Potato Chip plant currently located on the site and the construction of five multi-family residential structures, each with 24 units, for a total of 120 units on site. In addition, the plan shows an outdoor swimming pool, and a clubhouse to be constructed on the site. The property will have the parking for 180 vehicles on site. The Public Hearing was continued to March 2, 2000 and March 16, 2000 and closed on March 16, 2000. At a regularly scheduled meeting of the Planning Board held on March 16, 2000, the Board voted by a vote of eight (8) in favor, none (0) opposed, to approve the application as complying with the requirements of Site Plan Review, subject to the following conditions: 1. Conformance with the Plan Work shall conform with the set of plans entitled, "Ridgeside Apartments, Site Development Plans, Salem, MA", sheets 1-11, prepared by H.W. Moore Associates, Inc. dated December 27, 1999, last revised March 23, 2000. 2. Amendments Any amendments to the site plan shall be reviewed by the City Planner and if deemed necessary by the City Planner, shall be brought to the Planning Board. Any waiver of Ridgeside Apartments f March 16,2000-2 conditions contained within shall require the approval of the Planning Board. 3. Landscaping a. All landscaping shall be done in accordance with the approved set of plans. b. Trees shall be a minimum of 3 11/Y'caliper. c. Maintenance of landscape vegetation shall be the responsibility of the developer, his successors or assigns. d. After such landscaping is completed, the developer will add further landscaping if such is requested by the Planning Board or the City Planner. e. A landscaping irrigation system shall be installed throughout the landscaped areas of the site. f. The applicant, his successors, or assigns shall construct a wire mesh barrier, approximately four feet in height, which shall be weaved within the landscaping located adjacent to Highland Avenue. The wire mesh barrier will be utilized to deter visitors to enter the site through the landscaping. Final design of the barrier shall be approved by the City Planner. 4. Signage a. Proposed signage shall be reviewed and approved by the City Planner and the Sign Review Committee. b. The signage located at the entrance to the site shall be constructed of wood with carved lettering. 5. Lighting a. Lighting shall be installed as shown on the approved plan. b. No light shall cast a glare onto adjacent parcels or adjacent rights of way. c. A final lighting plan shall be submitted to the City Planner for review and approval prior to the issuance of a building permit. e. After installation, lighting shall be reviewed by the City Planner, prior to the issuance of a Certificate of Occupancy. 6. Construction Practices All construction shall be carried out in accordance with the following conditions: a. No work shall commence before 8:00 AM on weekdays and Saturdays. No work shall continue beyond 5:00 PM on weekdays and Saturdays. There shall be no work conducted on Sundays or holidays. Inside work of a quiet nature may be permitted at other times. Ride i g s de Apartments f - March 16,2000-3 b. All reasonable action shall be taken to minimize the negative effects of construction on abutters. Advance notice shall be provided to all abutters in writing at least 72 hours prior to commencement of construction of the project. c. Drilling and blasting shall be limited to Monday-Friday between 8:00 AM until 5:00 PM. There shall be no drilling or blasting on Saturdays, Sundays, or holidays. Blasting shall be undertaken in accordance with all local and state regulations. d. All construction vehicles shall be cleaned prior to leaving the site so that they do not leave dirt and/or debris on surrounding roadways as they leave the site. e. All construction shall be performed in accordance with all rules, regulations and ordinances of the City of Salem. f. All construction vehicles left overnight at the site, must be located completely on the site and not on First Street or Highland Avenue. 7. Clerk of the Works A Clerk of the Works shall be provided by the City, at the applicant, his successors or assigns' expense, as is deemed necessary by the City Planner. 8. Board of Health All Board of Health conditions as stated in the attached letter dated March 10, 2000. 9. Fire Department All work shall comply with the requirements of the Salem Fire Department. 10. Building Inspector All work shall comply with the requirements of the Salem Building Inspector. 11. Maintenance a. Refuse removal, ground maintenance and snow removal shall be the responsibility of the developer, his successors or assigns. b. Winter snow in excess of snow storage areas on the site shall be removed off site. c. Maintenance of all landscaping shall be the responsibility of the applicant, his successors or assigns. The applicant, his successors or assigns, shall guarantee all trees and shrubs for a two-(2) year period. 12. Postal Service All postal service, both pickup and delivery, shall be conducted in the clubhouse area. 13. Emergency Exit on Highland Avenue The emergency exit from the site located adjacent to Highland Avenue shall be constructed Ridgeside Apartments i March 16,2000-4 i with slant curbing and a grassed surface. If it is determined by the City Planner that residents and visitors to the site are utilizing the exit for vehicular purposes, the applicant his successors or assigns shall install a gate at the entrance, which will only be utilized by emergency service vehicles. 14. Dumpsters The height of the dumpster shall not exceed the height of the fenced enclosure for all dumpsters located on the site. 15. First Street a. The applicant,his successors, or assigns shall patch the sidewalk, located on First Street adjacent to the parcel and as shown on the approved site plan, as necessary. In addition the applicant, his successors, or assigns, shall overlay the sidewalk to the entry way for the development, which is located on First Street, with a minimum of 1'/s inches of pavement. b. The applicant, his successors, or assigns shall lay vertical granite curbing along the parcels' First Street frontage from the corner of First Street and Highland Avenue to the First Street entrance way to the site, as shown on the approved site plan. In addition, vertical granite curbing shall be laid for approximately three feet after the First Street entrance way to the site. 16. As-built Plans As-built plans, stamped by a Registered Professional Engineer, shall be submitted to the Planning Department and Engineering Department prior to the issuance of the final Certificate of Occupancy. 17. Violations Violations of any condition shall result in revocation of this permit by the Planning Board, unless the violation of such condition is waived by a majority vote of the Planning Board. I hereby certify that a copy of this decision and plans has been filed with the City Clerk and copies are on file with the Planning Board. This decision shall not take effect until a copy of this decision bearing the certification of the City Clerk that twenty (20) days have elapsed and no appeal has been filed or that if such appeal has been filed, and it has been dismissed or denied, is recorded in the Essex South Registry of Deeds and is indexed under the name of the owner of record is recorded on the owner's Certificate of Title.
