Curtin, Nersasian, Monaldo, St. Pierre, and Kasis OII Y 100 Highland Avenue Realty Trust 100 Highland Avenue Salem, MA 01970 On Thursday, September 18, 1986, the Planning Board of the City of Salem held a public hearing under Section VII—R, Site Plan Review, of the Salem Zoning Ordinance on the application of Drs. Curtin, Nersasian, Monaldo, St. Pierre, and Kasis, 100 Highland Avenue Realty Trust to construct a 12,020 square foot medical office building at 100 Highland Avenue. At a regularly scheduled meeting of the Planning Board held on November 20, 1986, the Board voted, by unanimous vote, to approve the application as complying with the requirements for issuance of a Special Permit , subject to the following conditions: 1 . Work shall conform to plan entitled, "Site Utility and Grading Plan, 100 Highland Avenue Realty Trust", prepared by M. A. Lally Associates, North Andover, MA. 2. Utility installation shall be approved by the Engineering Department. a. A Safety grate shall be installed on 24" culvert at the headwall located on the northwesterly side of the site. 3. Board of Health requirements set forth in a letter dated September 17 , 1986, shall be strictly adhered to. 4. Landscaping of the site shall conform to plan entitled, "Landscaping Site Plan for 100 Highland Avenue Realty Trust," prepared by Barlow Gillfillen Architects, dated November 7, 1986, revised November 18, 1986. a. Maintenance of landscaping in a healthy and thriving condition shall be the responsibility of the applicants, their e successors or assigns. b. There shall be no storage of snow on landscaped areas of the site. c. Location of granite curbing shall be reviewed and approved by the City Planner prior to issuance of a Certificate of Occupancy. Page 2 5. Board of Appeal decision dated May 28, 1986 shall be adhered to. 6. All construction shall be conducted in accordance with the following conditions: a. No work shall commence before 7:00 a.m. on weekdays and 8:00 a.m. on Saturdays. No work shall continue beyond 5:00 p.m. No work shall be conducted on Sundays or holidays. b. All reasonable action shall be taken to minimize the negative effects of construction on abutters. 7. Signage for the building shall be reviewed and approved by the City Planner prior to issuance of Certificates of Occupancy. 8. Any violation of these conditions will result in revocation of this permit by the Planning Board. At a regularly—scheduled meeting of the Planning Board held on June 25, 1986, the Board voted, by a vote of seven in favor, none opposed, to approve the application as complying with the requirements for the issuance of the Special Permit with the following conditions: 1 . Work shall conform to plan entitled, "Site Plan — Highland Avenue Salem, Mass." dated January 22, 1986, revised March 28, 1986. 2. Underground storage tanks, of the prior use, a gasoline station, shall be emptied and removed under the direction of the Fire Department. n shall be reviewed and approved b 3. All exterior lighting shall be installed and maintained in such a manner so as to not reflect or cause glare on abutting or facing residential premises or on—coming traffic. 7. The proposed access to the parking lot shall be one—way -- ingress shall be from Colby Street and egress shall be to Highland Avenue. 8. All construction shall be carried out in accordance with the Rules and Regulations of the Planning Board. 9. As—built plans, stamped by a Registered Professional Engineer or Land Surveyor shall be submitted to the Engineering Department prior to issuance of Certificates of Occupancy. 10.
