I hereby certify that, pursuant to Massachusetts General
Laws, the Salem Planning Board has taken the following action
with respect to the above-referenced subdivision :
By a vote of six in favor, none opposed, the Planning Board
approved the proposed subdivision plan as presented with the
attending conditions .
Notice of the Planning Board' s action and the reasons
supporting it are more fully describe in the attached copy of a
letter, the original of which was sent to the applicant by
registered mail , postage prepaid at his address stated on the
petition .

Dear mr. Brett:
The Salem Planning Board voted on February 15, 1990, to approve the proposed subdivision
plan entitled "Definitive Subdivision Plan - Greenlawn and Glen Avenue," dated February 14,
1989, revised December 26, 1989, subject to the following conditions:
1. Conformance with Plan
Work shall conform to plan entitled "Definitive Subdivision Plan - Greenlawn & Glenn
Avenue" dated February 14, 1989, revised December 26, 1989.
2. Utilities
Utility installation sliail be reviewed and approved by the City Engineer.
3. Board of Health
Board of Health requirements set forth in a letter dated November 14, 1989, shall be strictly
adhered to.
4. Clerk of the Works
A Clerk of the Works shall be provided at the expense of the developer as is deemed
necessary by the City Planner.
5. Tri Party Agreement
A Tri Party Agreement shall be filed with the Planning Board prior to release of the plan to
ensure completion of improvements required under the City of Salem Subdivison
Regulations, and as required by this approval.
6. Sidewalk/Planting Strip
A five foot sidewalk and a three foot planting strip shall be constructed along one side of
Greenlaw Avenue.
7. Trees
a. Trees shall be a minimum of 3 z" caliper, planted 30 feet on center.
1). Maintenance of landscape vegetation shall be the responsibility of the developer, his
successors or assigns.
8. Granite Curbing
6" vertical granite curbing shall be installed on both sides of street. However, the Planning
Board will consider eliminating the curbing requirement on the side of the street opposite
the proposed dwelling units if the developer works out language with the City Solicitor and
the language is adopted by the Planning Board relative to ensuring completion of the curbing
on the other side of the street.
9. Street Lighting
Street lighting shall be reviewed and approved by the City Electrician.
10. Construction Practices
All construction shall be carried out in accordance with the following conditions:
a. No work shall commence before 8:00 a.: ,. .;n weekdays and Saturdays. No work shall
continue beyond 5:00 p.m. There shall be nu work conducted on Sundays or holidays.
Inside work of a quiet nature may be permitted at other times.
1,. .�ll reasonable action shall be taken to minimize the negative effects of construction on
.;butters. Advance notice shall be provided to all abutters in writing at least 72 hours
prior to commencement of construction.
c. Drilling and blasting shall be limited to Monday Friday between 8:00 a.m. and 5:00 n.m.
There shall be no drilling or blasting on Saturdays, Sundays or holidays.
d. .=.11 construction vehicles shall be cleaned prior to leaving the site so that they do not
leave dirt and/or debris on roadways as they leave the site.
e. -_11 .:onstruction shaii be carried out in accordance with the Rules and Regulations of the
Planning Board.
11. bLnit Plans
As-built plans, stampea by a Registered Protessiunal Engineer or Land Surveyor, shall be
submitted to the Planning Department and Engineering Department prior to issuance of
Certificates of Occupancy.
12. `:iolations
Violations of any conditions shall result in revocation of this permit by the Planning Board.
his endorsement shall not take effect until a copy of the decision bearing the
certification of the City Clerk that twenty (20) days have elapsed and no appeal has been
£ilea or that if such appeal has been filed and it has been dismissed or denied, is recorded
in the Essex County Registry of Deeds and is indexed in the grantor index trader the name
of the owner of record or is recorded and noted on the owner's certificate of title. The
fee for recording or registering shall be paid by owner or applicant.
