(One �xlem Oreen DECISION Site Plan Review Special Permit SALEM ARMORY Museum Cooperative c\o Attorney John Serafini , Sr . 63 Federal Street Salem, MA 01970 On Thursday, September 19, 1991, the Planning Board of the City of Salem held a public hearing regarding the application of Museum Cooperative, c\o Attorney John Serafini , Sr . , 63 Federal Street , Salem, MA for the rehabilitation and reuse of the Salem Armory located at 136 Essex Street . At a regularly scheduled meeting of the Planning Board held on September 19, 1991, the Board voted by a vote of five in favor , none opposed, to approve the application as complying with the requirements of a Special Permit, subject to conditions : 1 . Conformance with Plan Work shall conform to plan entitled "Salem Center , Museum Master Plan-Site Plan" prepared by Dyer/Brown & Associates, dated 8\30\91, or as amended by the Salem Redevelopment Authority or its Design Review Board (DRB) . 2 . Planning Board Representatives to DRB a. The Planning Board shall designate two representative to continue to meet with the (DRB) on the progress of the project. b. The Planning Board designees along with the Developer shall report back to the Planning Bc•ard following the DRB approval and process. 3 . Amendments Any amendments to site plan shall require the approval of the Planning Board. 4. Utilities Utility installation shall be reviewed and approved by the Director of Public Services. 5. Board of Health Board of Health requirements outlined in a letter dated September 10, 1991, and attached to these conditions shall be strictly adhered to. 6 . Lighting Exterior lighting shall be installed and maintained in such a manner so as to not reflect or cause glare on abutting properties, or which adversely effect safe vision of operators of vehicles moving on nearby streets. 7 . Landscaping Landscaping and site development\furniture shall be subject to the review and approval of the Planning Department . 8 . Signage Signage shall be reviewed and approved by the SRA, DRB, and City Planner . 9. Maintenance a. Employ an acceptable method or means for the holding and disposal of trash (rubbish) after site development with a copy of this method sent to the Planning Department and Health Department in writing for their approval. b. Refuse removal, ground maintenance and snow removal shall be the responsibility of the developer, his successors or assigns. d 10 . Construction Practices All construction shall be carried out in accordance with the following conditions: a. No work shall commence before 8 : 00 a.m. on weekdays and Saturdays. No work shall continue beyond 5:00 p.m. There shall be no work conducted on Sundays or holidays. Inside work of. a quite nature may be permitted at other times. b. All reasonable action shall be taken to minimize the negative effects of construction on abutters . Advance notice shall be provided to all abutters in writing at least 72 hours prior to commencement of construction. C. All construction shall be performed in accordance with with the Rules and Regulations of the Planning Board, and in accordance with any and all rules, regulations and ordinances of the City of Salem. 11. Violations Violations of any conditions shall result in revocation of this permit by the Planning Board. This endorsement shall not take effect until a copy of this decision bearing certification of the City Clerk that twenty ( 20 ) days have elapsed and no appeal has been filed, or that if such appeal has been filed, and it has been dismissed or denied, is recorded in the Essex County Registry of Deeds and is indexed in the grantor index under the name of the owner of record or is recorded and noted on the owner ' s Certificate of Title. The fee for recording or registering shall be paid by the owner or applicant. BLENKHORN Salem, Massachusetts 01970 HEALTH AGENT 508-741-1800 September i0, 1991 Salem Arm&ry 136 Essex Street Salam, MA 01970 Board of Health Recommended Comments for Museum Cooperative -The design proposal concurs with the City Engineering Department and Cf .y Plumbing Inspector for utility tie-ins for drinking water, sanitary waste (sewage) , gas lines, and/or any other utility service. -Prior to water service (drinking water) being put into service it be tested for pressure and bacteria by a certified laboratory and the results submitted to the Planning and Health Departments in writing. -Employ an acceptable method for removal of all construction debris, and special waste (i.e. asbestos, etc. ) from the construction site. -Employ an acceptable method for dust and street cleaning control during site construction. -Prior to site start-up and demolition of existing buildings and also during site construction, employ a licensed pest control firm for site evaluation and service with a copy of the service program sent to the Planning and Health Departments. -All backfilling be done in accordance with good engineering practices to prevent future damage to all underground utilities. -Employ an acceptable method or means for the holding and disposal of trash (rubbish) after site development - suggestion/recommendation is inside trash compaction and outside removal of trash in self contained sealed containers.
