The Salem Planning Board voted on September 4, 2003 to endorse "Approval Under Subdivision
Control Law Not Required' on the following described plan:

2. Location: This lot is located at 12 Buena Vista Avenue (west), Assessors Map 9, and
Parcel 93.
3. Project Description: This Form A divides the lot know as "12 Buena Vista Avenue" and
creates two separate lots, 57A (15,036 sq. ft.) and 57B (86,990 sq ft.), as shown on the
plans titled, "Road Construction Plan, Buena Vista Ave. (West), dated September 30,
2003 and revised October 27, 2003, and the plan entitled "Easement Plan of the Land
