On Thursday, April 18, 1991, the Salem Planning Board held a public hearing under
Section VII-P Wetlands and Flood Hazard District Special Permit of the City of Salem
Zoning Ordinance for the construction of a single family dwelling at 79 Almeda Street,
within a wetland buffer zone.
At a regularly scheduled meeting of the Planning Board held on April 18, 1991, the
Board voted by a vote of seven in favor, none opposed, to approve the application as
complying with the requirements of a Special Permit, subject to the following conditions:
1. Conformance with Plan
Work shall conform to plan entitled "Plan of Land to Accompany a Notice of Intent,"
prepared by Hancock Survey Associates, Inc., dated March 25, 1991.
2. Conservation Commission
All conditions set forth in Order of Conditions $64-196 issued by the Salem
Conservation Commission, dated April 11, 1991, shall be strictrly adhered to.
3. Board of Health
Board of Health requirements stated in a letter dated April 11, 1991, shall be
strictly adhered to.
4. Construction Practices
All construction shall be performed in accordance with the Rules and Regulations of
the Planning Board, and in accordance with any and all applicable City of Salem
regulations.
5. Posting
Planning Board and Conservation Commission approvals shall be posted in a visible
location on the site throughout construction.
6. Violations
Any violations of these conditions shall result in revocation of this permit by the
Planning Board.

Clerk and copies are on file with the Planning Board. The Special Permit shall not take
effect until a copy of this decision bearing the certification of the City Clerk that twenty
(20) days have elapsed and no appeal has been filed or that if such appeal has been filed,
and it has been dismissed or denied, is recorded in the Essex County Registry of Deeds and
is indexed under the name of the owner of record or is recorded on the owner's Certificate
of Title. The fee for recording or registering shall be paid by the owner or applicant.
