At a regularly scheduled meeting of the Planning Board held on April 18, 1991, the Board voted by a vote of seven in favor, none opposed, to approve the application as complying with the requirements of a Special Permit, subject to the following conditions: 1. Conformance with Plan Work shall conform to plan entitled "Plan of Land to Accompany a Notice of Intent," prepared by Hancock Survey Associates, Inc., dated March 25, 1991. 2. Conservation Commission All conditions set forth in Order of Conditions $64-196 issued by the Salem Conservation Commission, dated April 11, 1991, shall be strictrly adhered to. 3. Board of Health Board of Health requirements stated in a letter dated April 11, 1991, shall be strictly adhered to. 4. Construction Practices All construction shall be performed in accordance with the Rules and Regulations of the Planning Board, and in accordance with any and all applicable City of Salem regulations. 5. Posting Planning Board and Conservation Commission approvals shall be posted in a visible location on the site throughout construction. 6. Violations Any violations of these conditions shall result in revocation of this permit by the Planning Board.
