DECISION s � s = • eaa August 2, 15 SPECIAL PERMIT WETLANDS AND FLOOD HAZARD SPECIAL PERMIT Mr. George Seeley Director of Engineering Services The Fafard Companies 290 Eliot Street Ashland, MA. 01721 On Thursday, July 18, 1985, the Planning Board of the City of Sales held a public hearing regarding the application of the Fafard Companies for Special Permits, under Sections P. Wetlands and Flood Hazard Special Permits, of the Salem Zoning Ordinance, with respect to a proposal to construct a 170,000 square fooc bulding to house Maynard Plastics Company, on a 13-acre parcel on Swampscott Road, Salem. (A portion of Assessors' Map 7, Lot 63) . At a regularly-scheduled meeting of the Planning Board held on August 1 , 1985, the Board voted, by a vote of six in favor, none opposed, one abstention, to approve the application as complying with the requirements for the issuance of the permit with the following conditions: BUSINESS PARK/SITE PLAN REVIEW Recuirements for Pe". its Compliance } 1 . All sedimentation and erosion control structures shall be installed at the 100' buffer zone line as determined by the Conservation Commission prior to commencement of work. Such structure shall be maintained during ing construction. 2. Stockpiled fill material excavated during the 21—E process which has been deemed to be hazardous shall be removed from the site within four weeks from the date of this decision. This material shall be removed to an approved facility and copies of delivery slips shall be supplied to the Planning Board. Until such material is removed, stockpiled material shall be covered and surrounded by hay bales. 3. A detail of silt fence to be installed shall be provided to the Planning Board. 4• .. -The proposed project in full shall be subject to the review and approval of the Conservation Commission. S. Roadway layout as approved by the Planning Board shall be strictly adhered to. Construction vehicles shall follow roadway layout to minimize impact upon wetlands. 14. Water and sewer designs shall be subject to the review and approval of the Engineering Department and Planning Board. 15. The number of parking spaces shall be conditional upon an analysis by the Planning Department of Maynard Plastics parking needs. Reserve spaces for additional parking shall be delineated on the approved plan. 16. The development to take place on the parcel shall be subject to the NEPA process. 19. Proposed signs shall be subject to the review and approval 'of the Planning Department. — Page 4 _ 20. Adequacy of culverts on Swampscott Road shall be analyzed by the developer-and the Planning Department. Upgrading of culverts, if deemed necessary, shall be undertaken by the developer at the cost of developer. the 21 . Fire access to all four sides of the proposed buildings shall be provided or requested by the Fire Department. 22. A lighting plan shall be subject to the review and approval of the Planning Board. 23. A landscaping plan shall be subject to the review and approval of the Planning Board. 24. Easements for access purposes to adjoining parcels in the entire 43 acre parcel as defined by the City shall be provided in a form approved by the Planning Board and City Solicitor. 25. The future roadway improvement needs of Swampscott Road shall be analyzed by the Planning Board and-,PlanningDepartment and needs as defined by such analysis shall be adhered to by the developer in the form of reservation of land necessary for such improvements. 26. A detailed specification of the storage tank areas for Maynard Plastics shall be provided to the Planning Board. Such detailed plans shall include contents of tanks and emergency procedures in the event of electrical failure. 27. A specification of the proposed control structure shall be supplied to the Engineering Dept. and Conservation Commission for review and approval . 18. Adequacy of fire flows and location of hydrants shall be approved by Fire Department. 29. A policy for the maintenance and cleaning of surface drains shall 'be presented to the Board of Health. 30. Prior to Certificate of Occupancy, the water system shall be tested for pressure and bacteria. 31 . An acceptable means for the holding and disposal of rubbish shall be submitted to the Board of Health for review and approval . 32 An acceptable means of removal of construction debris and vegetation shall be employed. , 33. Prior co commencement of construction, a licensed pest control firm shall be employed for site evaluation. Copy of results shall be submitted to Board of Health. c -- Page S — 34.
