At the request of the Salem Health Dept. an inspection of the above
referenced property was conducted and the following violations were noted:
1. Door at second floor level leading to front porch must be removed or
made safe by means of a full size grill.
2. Gutters at side of house must be replaced or removed.
3. There is a footing missing under the carrying post at rear porch.
4. Broken windows must be repaired.
5. Rear & front step rails must have ballasters or rails no further than
9" apart.
6. Loose ballasters must be repaired at front door.
7. Bulkhead door should have lock on inside.
Please contact this office upon receipt of this notice so as to inform us
of your course of action. Failure to comply will result in the appropriate

noted that the third floor had been occupied without benefit of a Certificate of
Occupancy.
Please be advised that you are in violation of the Massachusetts State
Building Code, Section 119.0.1 (Certificate of use and occupancy) .
You have seven (7) days to vacate the third floor roans.

1 . Reducing a sewer stack, third floor without authorization from the
Plumbing Inspector.
2. Attaching fitting for third floor bathroom, with existing bathrooms
below.
3. Second floor bathroom not properly vented.
4 . Cover plumbing beneath flooring without being inspected first .
These problems must be addressed immediately for the safety and health of the
habitation of this building. Failure to do so will result in this office taking
legal action against you.
