A building permit, #870-98 was issued on October 5, 1998 for your property located at 6
Phelps Street. Six months have lapsed and this department has no record of any
building inspections being done to date.
State building code requires us to revoke any permits that have not been acted upon
after six (6) months.
Please contact this office within ten (10) days upon receipt of this letter to discuss this
matter.

On jobs invobvinB reinforced concrete work, inspection must be made after steel is in place and before concrete
is poured.
The Department reserves the right to reject, any work which has been concealed or completed without first
having Ix•en inslx•ctod amd approved by the lkgmrmnmt in ,a-cordnnro with the requirements of the various cedes.
Any deviation from thi• approved plans must be nuthorizcd by the approval of revised plans subject to the same
procedure established 6,r the examination of the ,riginil plans. An additional permit fce is also charged predicated
on the extent of the vtvintion from the original pleas.
Permits we not vrdid if construction work is not started within six months from date permit is issued.
Request for Final Inspection should be made by postcard or phone call to this department when the•
construction work is completed and heating apparatus has been instilled. Painting or decorating is not required
before the Final Building Inspection.
Find Inspection and certiticute of occupancy must be obtaned before cocupying building.
