Our office received a complaint regarding construction without a Building Permit at your property located
at 36 Perkins Street—Unit 1. The complaints were investigated on Friday,January 20, 2017; Said
property appears to be in violation of Massachusetts State Building Code requirements.
Under the provisions of 780 CMR, Section 104.6—Right of Entry, of the State Building Code, access to this property
must be granted for the purposes of this inspection. Please call this office upon receipt of this letter to schedule this
required inspection. If this property has rental units, these tenants must be notified in advance of this inspection, so
that access to these spaces may also be accomplished
This Required Inspection shall be conducted by this office on Friday,January 27, 2017 at 9:00 a.m.; failure to
respond to this notification will be construed as non-compliance, and as such an Administrative Search Warrant will be
sought, so as to allow the lawful inspection of this property.

Dear Mr. Worth:
Due to complaints to this office, I conducted an inspection on September 2, 1999 at the
above mentioned address.
Upon my inspection, I have found the back exit stairways not to be in compliance with
Section 1028.2 of the Massachusetts State Building Code in keeping and maintaining all
exit stairways and fire escapes.
We would also like to do a check of the inside of the building to check the means of
egress and fire codes.
If you feel you are aggrieved by this order, your appeal would be directed to the BBRS,
State Building Code Appeal Board.
Failure to respond to these orders within fifteen (15) days will result in legal action being
taken against you in Salem District Court.

Mr. Human , A letter was sent to you on August 5,2008 regarding an incident at your Perkins
street property that occurred on August 3,2008. I have enclosed a copy of the letter. You were
directed to pull permits to remove the illegal sleeping room and bathroom that had been created
in the basement. To date, we have no records of any permits being pulled. No appeal has been
received by the Board of Buildings ,Regulations and Standards either. Enclosed,find a 21-D
ticket for your non-compliance. Further non-compliance will result in additional fines and a
