Dear Mr. McCarthy:
In response to a complaint regarding the above referenced property an
inspection was conducted and the following violations noted:
1. The rear egress door on the second floor was blocked. The reason
given for the blocking of this door was to hold the door shut as there
is no lock.
2. The second egress on the ground level had no rails on the exterior
stairs and the stairs are in need of immediate repair.
3. The side egress on the third floor is blocked at the landing by an
appliance and is impeding the doorway.
4. The second floor deck is in a serious state of disrepair and is a
hazard to public safety, this must be corrected immediately and the
required building permits must be issued.
These are all serious code violations and must be corrected
immediately. You are hereby ordered to take the necessary steps to correct
these violations and to notify this office within seven (7) days of receipt
of this notice that these corrections have been made and the permit to
either demolish or repair the deck has been issued.

Neighborhood complaints that this structures is being used as a Rooming
/Lodging House. This is not an allowed use for this property under the Provisions of
780 CMR, Massachusetts State Building Code, Section 310.5, which disallows the

In response to a complaint, on December 27, 1984, accompanied by Health
Department Sanitarian Joseph Lubas, an inspection of the common areas and one dwelling
unit of your property was conducted with the following safety code violations noted:
1. There is presently a problem with infestation of roaches that must be addressed.
2. There is a tremendous accumulation of flamable storage in the front basement
that poses a serious fire hazard. This hazard must be reduced to the satisfaction of
the Salem Fire Prevention Bureau.
3. Storage under the front egress stairs must be discontinued.
4. The kitchen ceiling in the Geary apartment is in poor repair and in danger of
falling.
5. Stair treads are in poor repair at the second level of the rear stairway.
In view of the hazard to the public safety and welfare involved in these violations, a
good faith effort to correct the above must be started within twelve (12) hours of your
receipt of this notice.
Within thirty (30) days the following must be corrected:
1. All gas stoves will be solid piped with an accessible shut-off
2. A smoke detection system will be in place and operable to the satisfaction of the
Salem Fire Prevention Bureau.
3. All missing stairway and porch baulsters will be replaced.
4. Hand rails will be provided at the basement stairs.
5. Emergency lighting will be provided to illuminate all paths of egress.

As a result of an inspection this date of the premises , structure ,
open land area or vehicle owned, occupied or otherwise under your
control , the following recommendations are submitted and shall serve
as a notice of violation of fire laws . These recommendations are
made in the interest of fire prevention and to correct- conditions
that are or may become dangerous as Et fire hazard or are in violation
of law.
You are hereby notified to remedy said violations named below within
seven days of above, date.
Such further action will be taken as the law requires , for failure
to comply with the above requirements within the stipulated Lime .
(Reference : General Laws of Commonwealth of Massachusetts , Chapter 148 ,

As a result of damage caused by water during the storm of Dec. 13, 1983 #
it was necessary to disconnect several electrical fixtures and wires
on your property at 29 Perkins Street, Salem, Mass. These wires
and fixtures have apparently not been attended to as of this date,
and the tenant is occupying the structure with these violations still
existing. This is in violation of the Minimum Standards of Human
Habitation and are creating other fire hazards as a result. You are
therefore ordered to have the electrical wiring corrected at this
occupancy, by a licensed electrician, and to notify this office when
said work is completed. Failure to perform repairs will constitute a
violation of the Salem Fire Code and the State health Code , and
present a hazard to life and property.
It has also been determined that you have real estate consisting of
residential occupancies which are not protected by approved smoke
detectors. A Form #120 (Violation of Chapter 148, Section 26C ,
Mass. General Laws) for failure to provide required smoke detectors
is being issued this date. You shall be required to install approved
smoke detectors and obtain the required Certificate of Completion,
on or before February 13, 1984, or such action as the law requires
shall be initiated.

Dear Mr. Maresco,
In response to a complaint, on December 27, 1984, accompanied by Health
Department Sanitarian Joseph Lubas, an inspection of the common areas and one dwelling
unit of your property was conducted with the following safety code violations noted:
1. There is presently a problem with infestation of roaches that must be addressed.
2. There is a tremendous accumulation of flamable storage in the front basement
that poses a serious fire hazard. This hazard must be reduced to the satisfaction of
the Salem Fire Prevention Bureau.
3. Storage under the front egress stairs must be discontinued.
4. The kitchen ceiling in the Geary apartment is in poor repair and in danger of
falling.
5. Stair treads are in poor repair at the second level of the rear stairway.
In view of the hazard to the public safety and welfare involved in these violations, a
good faith effort to correct the above must be started within twelve (12) hours of your
receipt of this notice.
Within thirty (30) days the following must be corrected:
1. All gas stoves will be solid piped with an accessible shut-off
2. A smoke detection system will be in place and operable to the satisfaction of the
Salem Fire Prevention Bureau.
3. All missing stairway and porch baulsters will be replaced.
4. Hand rails will be provided at the basement stairs.
5. Emergency lighting will be provided to illuminate all paths of egress.

make-up air.
