from the South Essex Registry District;

hand representing the death of said Mitchell Georgilakis also known as Manuel
Georgilakis, intestate, on April 11, 1989 and the administration of his estate by the Probate and
Family Court,Essex Division, in said Commonwealth;
yand further.representing the death of Peter J. Georgilakis, testate, on September 22, 1999
Tl and the allowance of his will by the Probate and Family Court, Essex Division, in said
0. Commonwealth;

and further representing the death of Christine Skylar, in Washington on February 12,
d 1996 and the allowance of her foreign will by the Probate and Family Court, Essex Division, in
said Commonwealth;
and further representing that all of the heirs of Mitchell Georgilakis have executed deeds
of their interest in the property described in Certificate of Title No. 12922 to Helen A.
Georgilakis,
and further representing the death of said Helen A. Georgilakis, testate, on December 10,
2004, and the allowance of her will by the Probate and Family Court, Essex Division, in said
Commonwealth; Helen A. Georgilakis devised all of her interest in said property described in
Certificate of Title No. 12922 to her daughter Cassandra C. Georgilakis;
and praying that all of the deeds from the heirs of Mitchell Georgilakis to Helen A.
Georgilakis be registered and noted on Certificate of Title No. 12922, that said certificate be
cancelledand a new certificate be issued to Cassandra C. Georgilakis;
