RE: 14 Osborne Street, Salem, MA.
Dear Mr. Ward:
This office has received a complaint of an unsafe condition at the above
referenced property.
This office inspected the property and found unsafe conditions on a
second floor deck. There is a stockade fence section loose and unsafe on one
end of the deck, also, the deck does not have any guard rails. This is a
violation of the Massachusetts State Building Code, Section 123 (Unsafe
Structure) . To correct this violation, the stockade fence should be fastened
securely to the deck and a 36" guard rail should be installed along the open
side.
Violation penalties are as follows:
Anyone who shall violate a provision of this code shall be
pxiishable by a firms of not mse than are—thousand ($1,000.) collars
or by imprisonment for not more tnan one (1) year, or both,
for each violation. Each day during which any portion of a
violation continues shall constitute a separate offense.
You have twenty-four (24) hours upon receipt of this letter to begin
correcting these violations. Failure to comply with this order will result
in this office taking legal action against you.

Osborne Street is presently being occupied as a Lodging House without being
properly licensed as required by Massachusetts General Law, and without the
necessary special permit from the City of Salem Zoning Board of Appeal.
Because of the serious nature of these allegations I must insist that you
contact me at 744-4580 within forty-eight (48) hours of your receipt of this notice
to arrange for an inspection of your property.
