Dear Mr. St. Pierre:
In accordance with Section 107 of the Massachusetts State Building Code, this office has reviewed the completed
T-Mobile equipment installation at the above-referenced location. The purpose of the review was to establish
compliance of the completed project with the approved design plans entitled "BS200/Park Towers", Site No:
"4BS020013", Rev.1: Dated 1/14/19, by Advanced Engineering Group, PC, and applicable provisions of the
Massachusetts State Building Code/International Building Code.
It should be noted that this inspection was limited to the installation of proposed T-Mobile equipment only. It is my
opinion that any minor deviations from the design plans do not endanger the intended function of the facility or the
safety of the public. This letter is not intended to relieve any party responsible for building construction from meeting
the requirements imposed by contract or other means, including commonly accepted industry practices.
Based on our review, I, Marc Chretien, licensed professional engineer number 40313, hereby certify that to the best
of my knowledge, information and belief that the work has been done in accordance with the approved plans and
the provisions of the Massachusetts State Building Code/International Building Code.

I certify that I have observed the work associated with the construction of the
Town Fair Tire Center at 311 Highland Avenue. To the best of my knowledge,
information and belief, the work has been done in conformance with the
approved plans and the provisions of the Massachusetts State Building Code
and all other pertinent laws, rules and regulations of the City of Salem. The
building construction and site development conform to the provisions of 521
CMR, Architectural Access Board Regulations. The building is now ready for
occupancy.
