This Special Permit regarding a proposal to construct an extension to an existing culvert under Highland Avenue and fill .an existing wetland area, is subject to the following conditions: 1 . Work shall conform to plans entitled, "Plan of Land in Salem, MA. prepared to accompany a Notice of Intent proposing to fill land of Salem Lodge #218, Loyal Order of Moose and Crete REalty Trust" , dated 8/1/83, revised 10/10/83, prepared by Thomas E. Neve, #30138. 2. Fill material shall be stockpiled on site, at which time the Board of Health shall be notified and shall perform an inspection of the fill material prior to placement within one business day of the notification. 3. An itemized log containing an inventory of all fill materials including the volume, source, contents, name of delivery person, and truck license number shall be kept on site and shall be available for inspection by the Conservation Commission, their agents, or the Board of Health and/or their agents. 4. No filling shall take place until the pipe which runs under Highland Avenue to which an extension is proposed by this project is cleaned. 5. Gentlemen: On Thursday, August 12 , 1982, we had the opportunity of meeting with Mr. Thomas Clougherty and Mr. Vic Karian, from your Department for the purpose of reviewing a site where we propose to operate a Landfill. Said site in located in the City of Salem, on Highland Avenue and is conprised of 5 . 4 acres, more or less. We propose to landfill non-toxic wastes, primarily fly-ash, excluding flamable substances, but including special non-toxic wastes acceptable to your office and the Salem Board of Health. At this site visit we reviewed the topographic and wetland features of the area and walked the perimeter of the site. During our walk and afterwards at a discussion immediately following, all were in general agreement that the location, type of facility proposed, and the overall feasibility of the project seemedto be both economically acceptable to my client and overall acceptable to the DEQE. However, Mr. Cloug"erty did stipulate that if a facility is approved that it must include the latest provisions of design as required by his Department. One of the major requirements would be the installation of an impervious "liner" at the base of the landfill. This liner would be sloped to facilitate a future lechate collection system if needed. The final landfill would also require an imprrvious "cap" before final vegitation is placed. Mr. Clougherty also explained the procedures in proposing such a facility as prescribed under the General Laws Chapter 111 Section 150A. It is our intention to proceed with a Site Assignment request to the Board of Health and a subsequent filing with the Conservation Commission. If Local accaptamce if awarded, we shall file definitive Design Plans to the City and to your Department for final approval. We are currently scheduled to meet with the Conservation Commission on Thursday, October 14 , 1982, and intend to request for a Board of Health public hearing sometime thereafter. Water sampling and analysis shall be performed by a State-certified laboratory prior to commencement of work and at six-month intervals during the life of the project. Water testing shall include total suspended solids, dissolved solids, total solids, Ph, Ethyl Benzene, Benzene, Xylene, Toluene. . Results of these tests shall be submitted to the Commissionwithin thirty (30) days of the sampling. The Commission shall be notified of the sampling dates and an authorized represen- tative of the Commission shall be present at the time of the sampling. Samples shall be taken at the location of the proposed head wall. Based upon the results of the independent laboratory testing, the frequency of additional testing shall be determined by the Commission. 21.
