This Department has received a report of an attic apartment being added at the above-
mentioned address.
This Department has no record or building, electrical or plumbing permits.
Also, Zoning Board approval would have been required.
Therefore, per State Building Code 780 CMR, Section 115.6, you are directed to contact
my office within 10 days to arrange for an inspection.
Failure to comply with this order will result in a complaint being filed in Salem District
Court.
If you fell you are aggrieved by this order, your appeal is to the Massachusetts Board of
Building Regulations and Standards in Boston, Ma
